芪参益气滴丸联合运动训练对冠心病经皮冠状动脉治疗术后患者心肺功能及运动能力的影响

Effect of Qishen Yiqi Dropping Pills(芪参益气滴丸)Combined with Exercise Training on Cardiopulmonary Function and Exercise Ability of Patients with Coronary Heart Disease Undergoing PCI

目的探究芪参益气滴丸联合运动训练对冠心病经皮冠状动脉治疗(PCI)术后患者心肺功能及运动能力的影响。方法选取2019年6月—2020年12月该院收治的110例行PCI的冠心病患者为研究对象,随机分为对照组和观察组,每组各55例。对照组予以冠心病二级预防常规药物治疗+运动训练,观察组在对照组基础上增加芪参益气滴丸治疗。比较两组治疗前后中医证候积分、生活质量[西雅图心绞痛量表(SAQ)]、心肺功能[1 s用力呼气容积(FEV1)、最大通气量(MVV)、左心室射血分数(LVEF)、左心室短轴缩短率(FS)、心肺运动试验1 min后心率恢复(HRR1)]、运动能力[最大摄氧量(VO_(2max))、最大运动负荷(MWL)、6 min步行试验(6MWT)]及血浆细胞因子[白细胞介素-10(IL-10)、转化生长因子-β1(TGF-β1)、N-末端脑钠肽前体(NT-proBNP)]水平。结果观察组治疗后中医证候积分较对照组明显更低,SAQ评分则明显更高(P<0.05)。观察组治疗后FEV1、MVV、LVEF、FS、HRR1均明显高于对照组(P<0.05)。与对照组相比,观察组治疗后VO_(2max)、MWL、6MWT均明显更高(P<0.05)。观察组较对照组治疗后血浆IL-10、TGF-β1、NT-proBNP水平均更低(P<0.05)。结论芪参益气滴丸联合运动训练可有效改善冠心病PCI术后患者的临床症状,强化其心肺功能和运动能力,从而提升患者生活质量,下调血浆IL-10、TGF-β1、NT-proBNP水平可能是其作用机制之一。

Objective To explore the effect of Qishen Yiqi Dropping Pills(芪参益气滴丸)combined with exercise training on cardiopulmonary function and exercise ability of patients with coronary heart disease undergoing percutaneous coronary intervention(PCI).Methods A total of 110 patients with coronary heart disease who underwent PCI in the hospital from June 2019 to December 2020 were randomly divided into control group and observation group,55 cases in each group.The control group was given routine drug treatment and exercise training for secondary prevention of coronary heart disease,and the observation group was given Qishen Yiqi Dropping Pills on the basis of the control group.Before and after treatment,TCM syndrome scores,quality of life(SAQ),cardiopulmonary function[one second forced expiratory volume(FEV1),maximum ventilation volume(MVV),left ventricular ejection fraction(LVEF),short axis shortening rate(FS),heart rate recovery after cardiopulmonary exercise test for 1 minute(HRR1)],exercise capacity[maximum oxygen uptake(VO_(2max)),maximum exercise load(MWL),6 min walking test(6 MWT)]and plasma cytokines[interleukin-10(IL-10),transforming growth factor-β1(TGF-β1),N-terminal-pro brain natriuretic peptide(NT-proBNP)]were compared between the two groups.Results After treatment,the TCM syndrome scores of the observation group were significantly lower than those of the control group,while the SAQ score was significantly higher(P<0.05).After treatment,the levels of FEV1,MVV,LVEF,FS and HRR1 in the observation group were significantly higher than those in the control group(P<0.05).Compared with those of the control group,the levels of VO_(2max),MWL and 6 MWT in the observation group were significantly higher after treatment(P<0.05).The levels of IL-10,TGF-β1 and NT-proBNP in the observation group were lower than those in the control group(P<0.05).Conclusion Qishen Yiqi Dropping Pills combined with exercise training can effectively improve the clinical symptoms of patients with coronary heart disease after PCI,strengthen their cardiopulmonary function and exercise ability,so as to improve the quality of life of patients,and reducing serum levels of IL-10,TGF-β1 and NT-proBNP may be one of the mechanisms.
