酪酸梭菌活菌散治疗细菌性肺炎合并腹泻患儿的临床效果

摘要 目的探讨酪酸梭菌活菌散治疗细菌性肺炎合并腹泻患儿的临床效果。方法选择2018年10月至2020年10月于我院住院的132例细菌性肺炎合并腹泻患儿为研究对象,按照随机数字表法将其分为对照组和观察组,各66例。对照组给予常规支持对症治疗,观察组在对照组的基础上给予酪酸梭菌活菌散治疗。比较两组的临床疗效、临床症状好转时间、T淋巴细胞亚群水平、大便次数及大便性状评分。结果观察组的治疗总有效率为86.36%,显著高于对照组的71.21%,差异具有统计学意义(P<0.05)。观察组的腹泻缓解时间、食欲改善时间和大便恢复正常时间均短于对照组,差异具有统计学意义(P<0.05)。治疗前,两组的白细胞分化抗原3阳性(CD3^(+))、白细胞分化抗原4阳性(CD4^(+))、白细胞分化抗原8阳性(CD8^(+))比较,差异无统计学意义(P>0.05);治疗1周后,两组的CD3^(+)、CD4^(+)均高于治疗前,CD8^(+)均低于治疗前,差异具有统计学意义(P<0.05);治疗1周后,观察组的CD3^(+)、CD4^(+)均高于对照组,CD8^(+)均低于对照组,差异具有统计学意义(P<0.05)。治疗前,两组的大便次数及大便性状评分比较,差异无统计学意义(P>0.05);治疗后,两组的大便次数少于治疗前,大便性状评分低于治疗前,差异具有统计学意义(P<0.05);治疗1周后,观察组的大便次数少于对照组,大便性状评分低于对照组,差异具有统计学意义(P<0.05)。结论酪酸梭菌活菌散治疗细菌性肺炎合并腹泻,能有效调节肠道菌群平衡及免疫功能,改善患儿的腹泻症状,值得临床推广。 Objective To investigate the clinical effect of Clostridium butyricum live powder in the treatment of children with bacterial pneumonia complicated with diarrhea.Methods A total of 132 children with bacterial pneumonia complicated with diarrhea hospitalized in our hospital from October 2018 to October 2020 were selected as the research objects.According to the random number table method,the children were divided into control group and observation group,with 66 cases in each group.The control group was given routine support symptomatic treatment,and the observation group was given Clostridium butyricum live powder on the basis of the control group.The clinical efficacy,clinical symptom improvement time,T lymphocyte subsets levels,stool frequency and stool trait score were compared between the two groups.Results The total effective rate of treatment in the observation group was 86.36%,which was significantly higher than 71.21%in the control group,and the difference was statistically significant(P<0.05).The diarrhea relief time,appetite improvement time and stool return to normal time in the observation group were shorter than those in the control group,and the differences were statistically significant(P<0.05).Before treatment,there were no significant differences in cluster of differentiation 3 positive(CD3^(+)),cluster of differentiation 4 positive(CD4^(+))and cluster of differentiation 8 positive(CD8^(+))between the two groups(P>0.05);after 1 week of treatment,CD3^(+)and CD4^(+)in the two groups were higher than those before treatment,and CD8^(+)was lower than that before treatment,and the differences were statistically significant(P<0.05);after 1 week of treatment,CD3^(+)and CD4^(+)in the observation group were higher than those in the control group,and CD8^(+)was lower than that in the control group,the differences were statistically significant(P<0.05).Before treatment,there were no significant differences in stool frequency and stool trait score between the two groups(P>0.05);after 1 week treatment,the stool frequency of the two groups was less than before treatment,the stool trait score was lower than before treatment,and the differences were statistically significant(P<0.05);after 1 week treatment,the stool frequency in the observation group was less than that in the control group,the stool trait score was lower than that in the control group,and the differences were statistically significant(P<0.05).Conclusion Clostridium butyricum live powder can effectively regulate the balance of intestinal flora and immune function in the treatment of children with bacterial pneumonia complicated with diarrhea,and improve the diarrhea symptoms of children,which is worthy of clinical promotion.
