基于患者需求视角下的快速康复护理模式在老年甲状腺手术患者中的应用效果及对预后的影响

Application effect of rapid rehabilitation nursing mode based on perspective of patients’demands in elderly patients undergoing thyroid cancer surgery and its influence on prognosis

目的探究基于患者需求视角下的快速康复护理模式在老年甲状腺手术患者中的应用效果及对预后的影响。方法回顾性分析本院2019年4月至2021年4月收治的107例接受甲状腺癌手术的老年患者的临床资料,依据护理方式的不同将其分为研究组(59例,基于患者需求视角下的快速康复护理)和对照组(48例,常规护理)。比较两组患者的护理效果。结果研究组的首次下床时间、术后疼痛时间及住院时间均显著短于对照组,住院费用显著少于对照组(P<0.05)。干预后,两组的抑郁自评量表(SDS)、焦虑自评量表(SAS)评分均低于干预前,且研究组低于对照组(P<0.05)。干预后,两组的促甲状腺激素(TSH)水平显著低于干预前,游离三碘甲状腺原氨酸(FT_(3))、游离甲状腺素(FT_(4))水平显著高于干预前,且研究组优于对照组(P<0.05)。研究组的护理满意度显著高于对照组(P<0.05)。研究组的并发症总发生率为5.08%,显著低于对照组的20.83%(P<0.05)。结论基于患者需求视角下的快速康复护理模式应用于老年甲状腺癌手术患者中的效果显著,可改善负面情绪及预后,提升甲状腺功能及患者的护理满意度。

Objective To explore the application effect of rapid rehabilitation nursing mode based on perspective of patients’demands in elderly patients undergoing thyroid cancer surgery and its influence on prognosis.Methods The clinical data of 107 elderly patients undergoing thyroid cancer surgery admitted in our hospital from April 2019 to April2021 were retrospectively analyzed.According to the different nursing methods,the patients were divided into study group(59 cases,rapid rehabilitation nursing based on perspective of patients’demands)and control group(48 cases,routine nursing).The nursing effects of the two groups were compared.Results The first time out of bed,postoperative pain time and hospitalization time in the study group were significantly shorter than those in the control group,and the hospitalization cost was significantly less than that in the control group(P<0.05).After intervention,the scores of Self-Rating Depression Scale(SDS)and Self-Rating Anxiety Scale(SAS)in the two groups were lower than those before intervention,and those in the study group were lower than the control group(P<0.05).After intervention,the thyroid stimulating hormone(TSH)level in the two groups was significantly lower than that before intervention,the levels of free triiodothyronine(FT_(3))and free thyroxine(FT_(4))were significantly higher than those before intervention,and those in the study group were better than the control group(P<0.05).The nursing satisfaction of the study group was significantly higher than that of the control group(P<0.05).The total incidence of complications in the study group was 5.08%,which was significantly lower than 20.83% in the control group(P<0.05).Conclusion Rapid rehabilitation nursing mode based on the perspective of patients’demands on elderly patients undergoing thyroid cancer surgery has a significant effect,which can ameliorate negative emotions and prognosis,improve thyroid function and patients’nursing satisfaction.
