不同时间玻璃体切除术治疗开放性眼外伤所致视网膜脱离的效果

The efficacy of vitrectomy at different times in the treatment of retinal detachment caused by open ocular trauma

目的比较不同时间玻璃体切除术治疗开放性眼外伤所致视网膜脱离的效果。方法回顾性病例对照研究。分析洛阳博爱眼科医院2015年10月至2020年10月收治的100例(100眼)开放性眼外伤所致视网膜脱离患者的临床资料。根据伤后行玻璃体切除术时间分为两组:观察组51例(51眼)于伤后6 d内进行手术;对照组49例(49眼)于伤后7~14 d进行手术。术后随访3个月。比较两组术后视网膜复位率、视力及增生性玻璃体视网膜病变(PVR)等并发症情况。结果观察组视网膜复位率96.08%(49/51)高于对照组的81.63%(40/49)(χ^(2)=5.33, P=0.021);观察组视力提高率高于对照组(χ^(2)=6.43, P=0.011);观察组并发症发生率低于对照组(χ^(2)=5.33, P=0.021), 其中观察组PVR发生率低于对照组(χ^(2)=6.30, P=0.012)。结论开放性眼外伤所致视网膜脱离, 于伤后6 d内进行玻璃体切除术, 效果更为理想。

Objective To compare the efficacy of vitrectomy at different times in the treatment of retinal detachment caused by open eye trauma.Methods This was a retrospective case control study.The clinical data of 100 eyes of 100 cases with retinal detachment caused by open eye trauma admitted to Luoyang Boai Eye Hospital from Oct.2015 to Oct.2020 were retrospectively analyzed.Based on the time of vitrectomy after injury,they were divided into two groups:51 eyes of 51 cases in the observation group were operated within 6 days after injury;49 eyes of 49 cases in the control group were operated on 7-14 days after injury.The postoperative retinal reattachment rate,visual acuity and complications such as proliferative vitreoretinopathy(PVR)were compared between the two groups.Results The retinal reattachment rate in the observation group was 96.08%(49/51),which was higher than the 81.63%(40/49)in the control group(χ^(2)=5.33,P=0.021);the visual acuity improvement rate in the observation group was higher than that in the control group(χ^(2)=6.43,P=0.011);the incidence of complications in the observation group was lower than that in the control group(χ^(2)=5.33,P=0.021),and the incidence of PVR in the observation group was lower than that in the control group(χ^(2)=6.30,P=0.012).Conclusion For retinal detachment caused by open eye trauma,vitrectomy within 6 days after injury is more effective.
