摘要
目的比较应用CASIA2和PentacamHR测量近视者眼前节参数的差异性。方法回顾性病例对照研究。纳入2021年3月至4月在安徽医科大学附属合肥爱尔眼科医院就诊拟行屈光手术的近视者120例(120眼), 分别使用CASIA2和PentacamHR测量眼前节各有关参数进行比较。结果与PentacamHR相比, CASIA2测量的前表面曲率(Z=-7.53, P<0.001)和全角膜屈光力(Z=-8.89, P<0.001)较高, 中央角膜厚度较薄(t=-9.03, P<0.001), 角膜总高阶像差较小(Z=-8.47, P<0.001), 瞳孔直径较大(t=41.88, P<0.001), 前房深度较深(t=-4.33, P<0.001), 角膜水平直径较小(t=-4.19, P<0.001)。角膜球差(6 mm)测量结果差异均无统计学意义(t=0.93, P=0.352)。CASIA2和PentacamHR两种仪器测量前表面平均曲率(ρ=0.98, P<0.001)、全角膜屈光力(ρ=0.97, P<0.001)、中央角膜厚度(r=0.94, P<0.001)、前房深度(r=0.89, P<0.001)相关性好, 角膜球差(6 mm)(r=0.52, P<0.001)、瞳孔直径(r=0.40, P<0.001)、角膜水平直径(ρ=0.68, P<0.001)结果相关性较好, 而角膜总相差(4 mm)(ρ=0.21, P=0.021)相关性较差。结论 CASIA2和PentacamHR对于近视者前表面曲率、全角膜屈光力、瞳孔大小及角膜水平直径测量结果的一致性差, 角膜厚度、前房深度和球面像差测量结果一致性好, 但总角膜高阶像差一致性差。
Objective To comare the difference of anterior segment parameters between CASIA2 and PentacamHR measurement on myopes.Methods This was a retrospective case control study.From Mar.to Apr.2021,120 eyes of 120 myopes were examined for refractive surgery in Affiliated Hefei Aier Ophthalmology Hospital of Anhui Medical University.The relevant parameters of anterior segment were measured with CASIA2 and PentacamHR respectively.Results Compared with PentacamHR,CASIA2 had higher mean keratometry(Z=-7.53,P<0.001)and total corneal refractive power(Z=-8.89,P<0.001),thinner central corneal thickness(t=-9.03,P<0.001),smaller high-order aberrations(Z=-8.47,P<0.001),larger pupil diameter(t=41.88,P<0.001),deeper anterior chamber depth(t=-4.33,P<0.001)and smaller horizontal diameter of the cornea(t=-4.19,P<0.001).There was no significant difference in corneal spherical aberration in 6 mm area(t=0.93,P=0.352).There was a good correlation between CASIA2 and PentacamHR in terms of mean keratometry(ρ=0.98,P<0.001),total corneal refractive power(ρ=0.97,P<0.001),central corneal thickness(r=0.94,P<0.001),anterior chamber depth(r=0.89,P<0.001).The correlation of corneal spherical aberration in 6 mm area(r=0.52,P<0.001),pupil diameter(r=0.40,P<0.001)and horizontal corneal diameter(ρ=0.68,P<0.001)was good,but the correlation of total corneal aberration in 4 mm area(ρ=0.21,P=0.021)was poor.Conclusion CASIA2 and PentacamHR have poor consistency for myopes in mean keratometry,total corneal refractive power,pupil size and corneal horizontal diameter,good consistency in corneal thickness,anterior chamber depth and spherical aberration,but the consistency of total corneal high-order aberration is poor.
