飞行人员肢体疼痛的肌骨超声诊断治疗及VR/AR干预效果 被引量:1

摘要 目的分析肌骨超声在飞行人员肢体疼痛中的诊断价值、疗养院软伤治疗效果及虚拟现实技术(virtual reality,VR)/增强现实技术(augmented reality,AR)对易引起训练伤的不良动作行为的干预价值,尝试新技术对现有疗养设备的应有拓展所带来的诊断-治疗-干预纠治思路探索。方法选取2020年4月一2021年12月到大连康复疗养中心疗养体检鉴定的有肢体疼痛的飞行人员119例作为研究对象。利用高频超声技术进行检查诊断,并与核磁结果进行比较以评判其效果,利用软伤医院进行治疗,并依据肌骨超声进行治疗效果评判,利用VR/AR模拟训练场景,观察肢体疼痛的飞行人员动作习惯与损伤关联情况,进行矫治干预并对干预效果进行评判及肌骨超声评估。结果与核磁共振结果比较,119例接受肌骨超声检查的飞行人员中阳性者117例(98.3%),仅2例未检查出阳性(1.7%),差异无统计学意义(P>0.05)。119例飞行人员进行软伤治疗后,肌骨超声检查结果表明,症状改善率100%,治愈率26.5%。119例飞行人员借fflVR/AR模拟训练场景进行不良动作行为干预,无意识非良性习惯动作发生率从89.1%下降到了11.8%,有意识纠正动作发生率达到86.8%。不良动作行为VR/AR干预前后肌骨超声评估,差异有统计学意义(于=4.267~18.667,P<0.05)。结论肌骨超声在飞行人员肢体疼痛中的应用,为临床提供重要的影像诊断和康复评估信息;疗养院内的新技术旧设备拓展诊断-有针对性的治疗-VR/AR模拟场景不良动作矫正-肌骨超声评估的闭环疗养过程,具有较高的军事意义。 Objective To analyze the diagnostic value of musculoskeletal ultrasound in limb pain of aircrews,the therapeutic effect against soft tissue injuries under rehabilitation and the efficacy of interventions of VR/AR in improper actions that are likely to cause training injuries in order to explore the applicability of diagnosis-treatment-intervention.Methods A total of 119 aircrews with limb pain who were admitted to Dalian Rehabilitation Center between April 2020 and December 2021 were selected as the subjects.High-frequency ultrasound was used for examination and diagnosis before being compared with MRI results to evaluate its effect.These subjects were treated in a hospital for soft tissue injuries and the therapeutic effect was evaluated via musculoskeletal ultrasound,VR/AR was used to simulate training scenarios to observe the association between the habitual action and injuries among aircrews with limb pain and conduct correction interventions.Results Compared with MRI,117 cases(98.3%)were positive and only 2 cases(1.7%)were not positive,so the difference was of no statistical significance(P>0.05).After treatment of soft tissue injuries of the 119 aircrews,musculoskeletal ultrasonography showed that the rate of symptom improvement was 100%and the cure rate was 26.5%.These aircrews underwent interventions in their undesirable action using VR/AR simulated training scenarios,so that the incidence of unconscious undesirable action decreased from 89.1%to 11.8%,and the incidence of conscious corrective action reached 86.8%The difference was of statistical significance after VR/AR simulated training scenarios as was evidenced by the finding of musculoskeletal ultrasound(χ^(2)=4.267-18.667,P<0.05).Conclusion The application of musculoskeletal ultrasound in treating the limb pain of aircrews can deliver important information for clinical imaging diagnosis and for assessment of rehabilitation.The flow of diagnosis-targeted treatment-correction of undesirable action via VR/AR simulation scenarios-musculoskeletal ultrasound evaluation-is of high military value.
