从阴阳学说探析腹泻型肠易激综合征

An analysis of diarrhea-type irritable bowel syndrome based on the theory of Yin and Yang

文章从阴阳学说探寻腹泻型肠易激综合征的西医发病机制、中医病因病机及两者的联系和治疗原则,认为腹泻型肠易激综合征病因多责之于素体阳虚、情志不畅或思虑过度,诱因为感受外邪、饮食所伤。本病基本病机为脾肾阳虚,寒湿内盛。心肝火旺于上、脾肾阳虚于下,水火不济,寒热错杂,脏腑阴阳失衡,气机逆乱。治疗上予补益中焦,寒热并治,调畅气机,以达到阴平阳秘。

In this paper,the pathogenesis of diarrhea-type irritable bowel syndrome in Western medicine,the etiology and pathogenesis of TCM,and the relationship and treatment principles of the two were explored based on the theory of Yin and Yang(阴阳学说).It is considered that the etiology of diarrhea-type irritable bowel syndrome is mostly Yang-deficiency(阳虚)of the body,poor emotion or excessive thinking.The incentive is the damage from external pathogens and eating disorder.The basic pathogenesis of the disease is Yang-deficiency of spleen and kidney,and excess of cold and dampness.The heart and liver fire are flourishing in the upper part,and Yang-deficiency of the spleen and kidney is in the lower part,resulting in water-fire imbalance,the cold and heat are mixed,Yin(阴)and Yang(阳)in viscera are unbalanced,and Qi movement(气机)is disordered.In terms of treatment,it is necessary to tonify middle Jiao(中焦),treat both cold and heat,and regulate Qi movement,so as to achieve the purpose of the balance of Yin and Yang.
