刘培民教授治疗阳明热盛型骨癌痛经验探析

An analysis of Professor LIU Peimin’s experience in treating the Yangming fever-type of bone cancer pain

骨癌痛是恶性肿瘤晚期骨转移患者临床常见的一类疾病,以疼痛为主要临床表现,疼痛发作时间不定,痛势轻重不一,严重影响肿瘤患者的生活质量。骨癌痛中医辨证分型多以虚实分为两大类,可见肾精亏虚、气滞血瘀等证型。刘培民教授提出骨癌痛还可辨证为阳明热盛证,以清化阳明热邪、解毒通经止痛为治疗原则,以汉代名医张机(字仲景)《伤寒杂病论》中的白虎汤为基础方,自拟加味白虎汤用于治疗骨癌痛患者,临床取得了显著疗效。文章总结其治疗阳明热盛型骨癌痛的临证经验,为骨癌痛的中医治疗提供有益思路。

Bone cancer pain is a common clinical disease in patients with advanced bone metastases from malignant tumors.Pain is the main clinical manifestation.The onset time of pain is wncertain,and the degrees of pain are different,which seriously affects the quality of life of tumor patients.Bone cancer pain is mostly divided into two TCM syndrome differentiation types according to deficiency and excess,including the deficiency of kidney meridian type,the Qi(气)-stagnation and blood stasis type.Professor LIU Peimin proposes that bone cancer pain can also be identified as the Yangming(阳明)heat syndrome,and the treatment principles are clearing away Yangming heat pathogens,detoxification,dredging meridian,and pain relief.The Baihu decoction(白虎汤)from Shanghan Zabing Lun(《伤寒杂病论》)by ZHANG Zhongjing,a famous doctor in the Han Dynasty,is used to treat bone cancer pain,and has gained a significant clinical effect.Therefore,this article summarizes the clinical experience of the treatment of bone cancer pain of the Yangming fever type,and provides a beneficial idea for the TCM treatment of bone cancer pain.
