两种射频热凝术治疗原发性三叉神经痛的综合比较

Comprehensive Comparison of Two Kinds of Radiofrequency Thermocoagu⁃lation in the Treatment of Primary Trigeminal Neuralgia

目的探讨半月神经节射频热凝术与三叉神经周围支射频热凝术治疗原发性三叉神经痛的差别。方法选取滨州医学院附属医院于2013年1月—2014年6月收治的56例行周围支射频热凝术的患者(B组)及2014年7月—2016年12月收治的57例行半月神经节射频热凝术的患者(A组),比较两组患者术后5 d的临床疗效、并发症、住院天数、住院费用。结果术后5 d,A组临床治愈率为49.1%明显高于B组的17.9%,差异有统计学意义(χ^(2)=12.372,P<0.001);A组及B组患者住院天数为9(7,13)d和13(10.25,17)d,两组住院天数比较,差异有统计学意义(Z=-3.908,P<0.001);但A组患者住院费用8935.32(7718.215,10920.195)元,B组患者住院费用7762.97(6286.305,8772.9425)元,A组住院费用显著高于B组,差异有统计学意义(Z=-4.003,P<0.001);两组并发症发生率及临床有效率比较,差异无统计学意义(P>0.05)。结论半月神经节射频热凝术具有高临床治愈率,患者住院时间短,但花费高。

Objective To investigate the difference between radiofrequency thermocoagulation of semilunar ganglia and radiofrequency thermocoagulation of peripheral trigeminal nerve in the treatment of primary trigeminal neuralgia.Methods Fifty-six patients who underwent radiofrequency thermocoagulation of peripheral branches from January 2013 to June 2014(group B)and 57 patients who underwent radiofrequency thermocoagulation of semilunar ganglia from July 2014 to December 2016(group A)were collected from the Affiliated Hospital of Binzhou Medical College,to compare and analyze the clinical efficacy,complications,length of hospital stay,and hospitalization expenses 5 d after the operation between the two groups.Results The 5 d clinical cure rate in group A was 49.1%,and the 5 d clinical cure rate in group B was 17.9%.The clinical cure rate of group A was significantly higher than that of group B,and the difference was statistically significant(χ^(2)=12.372,P<0.001).The hospitalization days of patients in group A and group B were 9(7,13)d and 13(10.25,17)d,and there was a statistically significant difference in hospitalization days between the two groups(Z=-3.908,P<0.001).However,the hospitalization cost of patients in group A was 8935.32(7718.215,10920.195)yuan,and the hospitalization cost of patients in group B was 7762.97(6286.305,8772.9425)yuan.The hospitalization cost of patients in group A was significantly higher than that in group B,the differ⁃ence was statistically significant(Z=-4.003,P<0.001).There was no statistical difference in the incidence of compli⁃cations and clinical efficacy between the two groups(P>0.05).Conclusion Radiofrequency thermocoagulation of the semilunar ganglion has a high clinical cure rate and a short hospital stay,but the cost is high.
