糖化血红蛋白与血脂检测在老年2型糖尿病筛查中的应用探讨 被引量:2

摘要 目的 探讨糖化血红蛋白和血脂检测在老年2型糖尿病检测中的应用价值。方法 选取2020年6月—2021年6月山东省成武县人民医院收治的60例老年单纯2型糖尿病患者作为观察A组,选取同期于本院诊治的60例老年伴有微血管病变的2型糖尿病患者作为观察B组,选取同期于本院进行健康检查的60例未患有糖尿病的老年人作为对照组。比较3组糖化血红蛋白、总胆固醇、三酰甘油、高密度脂蛋白胆固醇、低密度脂蛋白胆固醇水平。结果 观察A组的糖化血红蛋白水平(9.01±1.56)%显著高于对照组(5.09±0.31)%,差异有统计学意义(t=19.091,P<0.05)。观察B组的糖化血红蛋白水平(10.21±1.64)%显著高于观察A组,差异有统计学意义(t=4.107,P<0.05)。观察B组的总胆固醇(TC)、三酰甘油(TG)、高密度脂蛋白胆固醇(HDL-C)、低密度脂蛋白胆固醇(LDL-C)依次为(5.29±0.63)、(2.18±0.69)、(0.92±0.51)、(2.90±0.68),与观察A组的(4.74±0.42)、(1.75±0.48)、(1.18±0.42)、(2.34±0.43)mmol/L比较,差异有统计学意义(t=5.627、3.963、3.048、5.392,P<0.05)。结论 2型糖尿病患者的糖化血红蛋白、血脂水平高于健康人群,而发展到出现微血管并发症的2糖尿病患者其糖化血红蛋白、血脂水平要高于未出现微血管病变的2型糖尿病患者。对老年人群开展糖化血红蛋白与血脂检测,能够帮助筛查2型糖尿病,并且还能够帮助评估糖尿病病情,判断是否已经发展到出现微血管病变的程度,为患者的诊治提供参考。 Objective To explore the application value of glycosylated hemoglobin and blood lipid detection in elderly type 2 diabetes mellitus.Methods 60 elderly patients with simple type 2 diabetes admitted to the People’s Hospital of Chengwu County, Shandong Province from June 2020 to June 2021 were selected as observation group A, 60elderly patients with type 2 diabetes with microvascular disease diagnosed and treated in the hospital in the same period were selected as observation group B, and 60 elderly patients without diabetes who underwent health examination in the hospital in the same period were selected as control group. The levels of glycosylated hemoglobin, total cholesterol, triacylglycerol, high-density lipoprotein cholesterol and low-density lipoprotein cholesterol were compared among the three groups.Results The glycated hemoglobin level of observation group(9.01±1.56)% was significantly higher than that of control group(5.09±0.31)%, and the difference was statistically significant(t=19.091, P<0.05).The glycated hemoglobin level(10.21±1.64)% in observation group B was significantly higher than that in observation group A, and the difference was statistically significant(t=4.107, P<0.05). The total cholesterol(TC), triacylglycerol(TG), high density lipoprotein cholesterol(HDL-C) and low density lipoprotein cholesterol(LDL-C) in group B were(5.29±0.63) mmol/L,(2.18±0.69) mmol/L,(0.92±0.51) mmol/L,(2.90±0.68) mmol/L, compared with the observation group A(4.74±0.42) mmol/L,(1.75±0.48) mmol/L,(1.18±0.42) mmol/L,(2.34±0.43) mmol/L, the difference was statistically significant(t=5.627, 3.963, 3.048, 5.392, P<0.05).Conclusion The levels of glycated hemoglobin and blood lipids in patients with type 2 diabetes are higher than those in healthy people, and the levels of glycated hemoglobin and blood lipids in patients with type 2 diabetes who develop microvascular complications are higher than those in patients with type 2 diabetes without microvascular disease. The detection of glycosylated hemoglobin and blood lipids in the elderly can help to screen for type 2 diabetes, and can also help to assess the condition of diabetes, determine whether it has developed to the degree of microvascular disease, and provide reference for the diagnosis and treatment of patients.
