基于证素辨证研究闽东地区不同年龄围绝经期综合征患者的中医证候特点

A study on TCM syndrome characteristics of perimenopausal syndrome patients at different age in Eastern Fujian based on syndrome-element differentiation

目的:初步探索闽东地区不同年龄围绝经期综合征患者的中医证候特点。方法:采用证素辨证的方法,根据《黄帝内经》“六七”“七七”的理论,评估不同年龄围绝经期综合征患者的中医证候分布。结果:围绝经期综合征的前5位病位证素依次为肾>肝>胞宫>脾>心;前9位病性证素依次为阴虚>气滞>热>气虚>湿>血瘀>痰>血虚>阳虚。其中49~56岁妇女肾、脾、气虚病理积分明显高于42~48岁。结论:肾虚肝郁是围绝经期综合征的主要中医病理,虚实夹杂是本病的基本病机;针对42~48岁围绝经期患者,应以重视补肾疏肝为主;49~56岁围绝经期患者,应兼顾健脾益气,化湿祛浊。

Objective:To preliminarily explore TCM syndrome characteristics of perimenopausal syndrome patient at different age in Eastern Fujian(闽东地区).Methods:According to the theory of“six-seven”and“seven-seven”in Huangdi Neijing(《黄帝内经》),the method of syndrome-element differentiation was used to evaluate the distribution of TCM syndromes in perimenopausal syndrome patients at different age.Results:The top 5 syndrome elements of disease location of perimenopausal syndrome were as follows:kidney>liver>uterus>spleen>heart.The top 9 syndrome elements of disease nature of perimenopausal syndrome were as follows:Yin(阴)deficiency>Qi(气)stagnation>heat>Qi deficiency>dampness>blood stasis>phlegm>blood deficiency>Yang(阳)deficiency.Among them,the pathological scores of kidney,spleen and Qi deficiency in women aged 49 to 56 were significantly higher than those in women aged 42 to 48.Conclusion:Kidney deficiency and liver depression is the main TCM pathology of perimenopausal syndrome,and the mixture of deficiency and excess is the basic pathogenesis of the disease.For perimenopausal syndrome patients aged 42 to 48,attention should be paid to tonifying the kidney and soothing the liver.For perimenopausal syndrome patients aged 49 to 56,consideration should be given to strengthening the spleen and replenishing Qi.
