基于数据挖掘的咽喉反流性疾病中医用药规律研究

Study on medication regularity of TCM in cases of laryngopharyngeal reflux disease based on data mining

目的:基于数据挖掘分析咽喉反流性疾病的中医用药规律。方法:搜集并筛选发表的与中医药治疗咽喉反流性疾病相关的文献,对证型、组方和药物信息进行提取、规范,构建数据库,使用古今医案云平台对证型、组方、高频药物、功效、性味和归经进行统计,并对药物进行关联规则分析、聚类分析和复杂网络分析。结果:本研究共纳入96篇文献,提取处方104首,药物146味,高频证型为痰气交阻证、肝胃不和证、脾胃虚弱证、肝郁脾虚证,高频组方为半夏厚朴汤、旋覆代赭汤、柴胡疏肝散、半夏泻心汤。高频用药为半夏、甘草、茯苓等22味,以燥湿化痰药和清热解毒为主,四气以温、平、微寒、寒为先,五味以苦、辛、甘为主,用药大多归肺经、脾经、胃经、心经、肝经。挖掘出12个药物关联规则、4个聚类组合和1个核心组方。结论:中医药治疗咽喉反流性疾病药物配伍上重在调和脏腑枢机,即通过调和肝、脾胃、肺等脏腑枢机以恢复脾胃枢机升降之用,同时予以利咽、化痰、止咳等治法以标本兼治。

Objective:To analyze the law of TCM medicine for laryngopharyngeal reflux disease(LPRD)based on data mining technology.Methods:To collect and screen the published literature related to the treatment of LPRD in TCM,extract and standardize the information on evidence types,group prescriptions and TCM,construct a database,and use the Ancient and Modern Medical Case Cloud platform to conduct statistics on evidence types,group prescriptions,high-frequency TCM medicine,efficacy,nature and ascription,as well as association analysis on TCM medicine,cluster analysis and complex network analysis.Results:A total of 96 papers were included in this study,with 104 prescriptions and 146 TCM medicine were extracted.The high-frequency evidence types were phlegm and Qi(气)obstruction,liver and stomach disharmony,spleen and stomach weakness,and liver-depression and spleen deficiency,and the highfrequency grouping of prescriptions were the Banxia Houpu decoction(半夏厚朴汤),the Xuanfu Daizhe decoction(旋覆代赭汤),the Chaihu Shugan decoction(柴胡疏肝散),and the Banxia Xiexin decoction(半夏泻心汤).The high-frequency medicines are 22 flavors such as pinellia,licorice,poria etc.,which are mainly used to dry dampness,resolve phlegm and clear heat and detoxify toxins.The four Qi are mainly warm,calm,slightly cold and cold,while the five flavors are mainly bitter,pungent and sweet.Twelve TCM association rules,four clustering combinations and one core group formula are mined.Conclusion:In the treatment of LPRD,the combination of drugs in TCM for LPRD focuses on harmonizing the internal organs,by harmonizing the internal organs of the liver,spleen,stomach and lung to restore the elevation of the central organs of the spleen and stomach,while at the same time treating the throat,resolving phlegm and relieving cough to treat both the symptoms and the root cause.
