消溶稳斑方对心绞痛后冠脉微循环障碍患者心肌灌注及一氧化氮、内皮素-1的影响

The effects of the Xiaorong Wenban prescription on myocardial perfusion, NO and ET-1 in patients with coronary microcirculation disturbance after angina pectoris

目的:分析消溶稳斑方对不稳定型心绞痛后冠脉微循环障碍患者心肌血流灌注情况及一氧化氮(Nitric Oxide,NO)、内皮素-1(Endothelin-1,ET-1)的影响。方法:选取2019年6月-2020年6月就诊于河南中医药大学第一附属医院的不稳定型心绞痛后冠脉微循环障碍(气虚血瘀痰阻证)患者68例,按照随机数字表法分为治疗组与对照组(各34例)。对照组予西医常规药物治疗,治疗组在西医常规治疗基础上加消溶稳斑方颗粒,两组疗程均为24周。对比两组患者治疗前后血心肌血流灌注情况及血清NO、ET-1变化。结果:治疗后两组患者心肌血流灌注、NO、ET-1均较治疗前显著改善(P <0.05);治疗后治疗组患者静息心肌血流量(Resting Myocardial Blood Flow,rMBF)、负荷心肌血流量(Stress Myocardial Blood Flow,sMBF)、心肌血流储备分数(Fractional Flow Reserve,FFR)均优于对照组(P <0.05);治疗后治疗组血清NO高于对照组,ET-1低于对照组(P <0.05)。结论:消溶稳斑方可有效改善不稳定型心绞痛后冠脉微循环障碍患者的心肌血流灌注,有效保护血管内皮功能。

Objective:To analyze the effects of the Xiaorong Wenban prescription(消溶稳斑方)on myocardial blood perfusion,NO and ET-1 levels in patients with coronary microcirculation disorder after unstable angina pectoris.Methods:Sixty-eight cases from May 2020 to March 2021 were selected.According to the random number table method,they were divided into the treatment group and the control group(34 cases for each).The control group was given conventional Western medicine,while the treatment group was treated with granules dosage of the Xiaoyu Wenban prescription more.The course of treatment in both groups was 24 weeks.Results:After treatment,myocardial blood perfusion,NO and ET-1 levels in the two groups were significantly ameliorated compared with before treatment(P<0.05).After treatment,the rMBF,sMBF and myocardial FFR levels in the treatment group were all better than those in the control group(P<0.05).After treatment,serum NO level in the treatment group was higher than the control group,and ET-1 level was lower than the control group(P<0.05).Conclusion:The Xiaorong Wenban prescriptioncan can effectively improve myocardial blood perfusion and protect vascular endothelial function in patients with coronary microcirculation disorder after unstable angina pectoris.
