股骨颈骨折患者全髋关节置换术中行关节囊修复的效果观察

Application of capsule repair in total hip arthroplasty for femoral neck fractures

目的探讨股骨颈骨折患者全髋关节置换术中行关节囊修复的临床效果。方法回顾性分析2017年1月至2019年12月在常熟市第一人民医院行全髋关节置换术的80例股骨颈骨折患者的临床资料,将术中行关节囊修复的40例患者的临床资料纳入A组,未行关节囊修复40例患者的临床资料纳入B组。术后随访3个月,比较两组患者围术期显性失血量及隐性失血量,比较两组患者术前及术后3个月的髋关节功能以及平衡功能。结果A组显性失血量及隐性失血量均少于B组,差异有统计学意义(P<0.05);术后3个月,两组Harris评分均较术前升高,且A组Harris评分高于B组,差异有统计学意义(P<0.05);术后3个月,两组Berg平衡量表评分均高于术前,且A组高于B组,差异有统计学意义(P<0.05)。结论在股骨颈骨折全髋关节置换术中进行关节囊修复,利于减少围术期显性及隐性失血量,且患者术后髋关节功能及平衡功能恢复效果更好。

Objective To investigate the clinical effect of joint capsule repair in total hip replacement on patients with femoral neck fractures.Methods The clinical data of 80 patients with femoral neck fractures who underwent total hip replacement in Changshu First People’s Hospital from January 2017 to December 2019 were retrospectively analyzed.The clinical data of 40 patients who underwent intraoperative joint capsule repair were included in group A,and the clinical data of 40 patients who did not underwent intraoperative joint capsule repair were included in group B.All the patients were followed up for 3 months,the perioperative dominant blood loss and hidden blood loss were compared between the two groups,and the hip joint function and balance function of the two groups before and 3 months after operation were compared.Results The dominant blood loss and hidden blood loss in the group A were lower than those in the group B,and the difference was statistically significant(P<0.05).Three months after operation,the Harris scores in both groups were higher than those before operation,and the Harris score in the group A was higher than that in the group B,and the difference was statistically significant(P<0.05).Three months after operation,the Berg balance scale(BBS)scores in both groups were higher than those before operation,and the BBS score in the group A was higher than that in the group B(P<0.05).Conclusions In total hip arthroplasty for femoral neck fractures,joint capsule repair is beneficial to reduce the perioperative dominant and hidden blood loss,and the recovery effect of hip joint function and balance function is better.
