唾液细胞因子和菌斑致病菌与老年患者牙周病状态的分析 被引量:3

摘要 目的:探究老年牙周炎患者唾液生物标志物和菌斑致病菌水平与牙周炎严重程度间的关系。方法:选择2018年6月-2021年3月首都医科大学附属北京口腔医院收治的牙周炎患者126例作为研究对象,选择126例健康者作为对照组。所有研究对象均进行口腔检查,收集所有研究对象的唾液,检测唾液中白细胞介素-1β(IL-1β)、白介素-6(IL-6)、白介素-8(IL-8)、白介素-10(IL-10)、肿瘤坏死因子-α(TNF-α)、基质金属蛋白酶-8(MMP-8)、基质金属蛋白酶-9(MMP-9)水平。收集研究对象的龈下菌斑,采用探针法实时荧光定量PCR(RT-qPCR)检测牙龈卟啉单胞菌(P.gingivalis)、福赛坦氏菌(T.forsyth ia)和齿垢密螺旋体(T.denticola)水平,进一步比较对照组、不同严重程度牙周炎患者间的水平差异。结果:研究组患者的附着丧失(AL)、探诊深度(PD)、探诊出血(BOP)阳性率均显著高于对照组;研究组患者IL-1β、IL-8、MMP-8、MMP-9水平均显著提高,且中重度组患者水平较轻度组患者更高;研究组患者P.gingivalis、T.forsythia和T.denticola菌量显著高于对照组,且中重度牙周病患者菌量较轻度组患者更高。结论:老年牙周炎患者唾液生物标志物水平、菌斑致病菌水平显著提高,且与疾病程度呈正相关。 Objective:To explore the differences between different disease states and the levels of salivary cytokines and plaque pathogen in elderly patients with periodontitis.Methods:126 patients with periodontitis admitted to Beijing Stomatological Hospital,Capital Medical University from June 2018 to March 2021 were selected as the objects,and 126 healthy subjects were selected as the controls.All subjects undergo oral examination,saliva were was collected to detect the levels of interleukin-1β(IL-1β),interleukin-6(IL-6),interleukin-8(IL-8),and interleukin-10 in saliva(IL-10),tumor necrosis factor-α(TNF-α),matrix metalloproteinase-8(MMP-8),matrix metalloproteinase-9(MMP-9).The subgingival plaque were collected,and real-time fluorescent quantitative PCR(RT-qPCR)was used to detect P.gingivalis,T.forsythia and Treponema denticola.(T.denticola)level,further compare the levels between the control group and patients with different severity of periodontitis.Results:The level of attachment loss(AL),probing depth(PD),and positive rates of probing bleeding(BOP)of the study group were significantly higher than those in the control group.The levels of IL-1β,IL-8,MMP-8,and MMP-9in the study group were significantly increased,and the level of patients in the moderate to severe group was higher.The bacterial counts of P.galivalis,T.Forsythia and T.Denticola in the study group were significantly higher than those in the control group,and the bacterial count was higher in patients with moderate to severe periodontal disease.Conclusion:The saliva biomarker level and the level of plaque pathogenic bacteria in elderly patients with periodontitis increased significantly,and was positively correlated with disease severity.
