血清嗜铬粒蛋白A在嗜铬细胞瘤和副神经节瘤中的应用价值

目的探讨血清嗜铬粒蛋白A(CgA)在嗜铬细胞瘤和副神经节瘤(pheochromocytoma and paraganglioma,PPGL)中的诊断及疗效评价的价值。方法选取2017年6月~2021年6月在北京核工业医院核素诊疗中心住院行131I-间位碘代苄胍(131I-metaiodobenzyl guaniodine,131I-MIBG)治疗的PPGL患者80例,采用双抗体夹心酶联免疫吸附试验(ELISA)检测患者血清CgA水平,评价CgA在转移性PPGL及进展期PPGL中的表达及意义。记录PPGL患者的临床病理特征(性别、年龄、肿瘤功能状态、有无远处转移),根据疗效评价结果,分为治疗有效组和治疗无效组,对两组CgA水平进行比较。结果PPGL患者血清CgA水平与患者的性别、年龄、肿瘤功能状态无关,差异无统计学意义(P>0.05)。PCC组与PGL组CgA水平相当,差异无统计学意义(P>0.05)。治疗无效组CgA水平明显高于治疗有效组,差异具有统计学意义(P<0.05)。结论血清CgA在诊断及评价进展期及转移性PPGL方面有一定的意义。

Objective To explore the disease diagnosis and evaluation value of serum chromaffin A(CgA)in pheochromocytoma and paraganglioma(PPGL).Methods A total of 80 patients with PPGL who were hospitalized and treated with 131I-metaiodobenzyl guaniodine(131I-MIBG)from June 2017 to June 2021 were selected in Beijing Nuclear Industry Hospital Radionuclide Diagnostic and Treatment Center.Serum CgA levels were detected by double-antibody sandwich enzyme-linked immunosorptive assay(ELISA),and the expression and signifi cance of CgA in metastatic and advanced PPGL were evaluated.The clinicopathological characteristics(sex,age,tumor functional status,and the presence or absence of distant metastasis)of PPGL patients were recorded,and the levels of CgA were grouped and compared.Results There was no signifi cant diff erence in serum CgA level in PPGL patients with diff erent gender,age,tumor site and tumor functional status(P>0.05).The level of CgA in PCC group and PGL group was similar,and there was no statistical signifi cance(P>0.05).The level of CgA in the ineff ective group was signifi cantly higher than that in the eff ective group,and the diff erence was statistically signifi cant(P<0.05).Conclusion Serum CgA determination has certain signifi cance in the diagnosis and evaluation value of disease progression and metastatic PPGL.
