Bacterial resistance surveillance in a hospital from 2018 to 2020

摘要 目的 分析2018-2020年某院常见临床分离菌的分布及耐药情况,为临床医师对抗菌药物的合理应用提供参考依据。方法 回顾性分析2018-2020年该院临床标本分离菌的分布及耐药性数据,依据美国临床和实验室标准化协会2019年文件标准判读药敏试验结果。结果 2018-2020年共检出细菌6 862株,其中革兰阳性菌840株(12.24%),革兰阴性菌5 863株(85.44%)。未检出耐万古霉素和利奈唑胺的葡萄球菌。仅检出少数耐万古霉素肠球菌。革兰阴性菌检出率居前4位者分别为肺炎克雷伯菌[19.67%(1 350/6 862)]、奇异变形杆菌[8.96%(615/6 862)]、大肠埃希菌[7.74%(531/6 862)]、黏质沙雷菌[2.93%(201/6 862)]。肠杆菌科细菌对β-内酰胺类、氨基糖苷类、碳青霉烯类耐药率均较低,鲍曼不动杆菌对亚胺培南耐药率较高(77.01%)。结论 进行细菌耐药性分析对医院抗菌药物的合理应用和科学管理具有重要意义,应加强细菌耐药性监测。 Objective To study the distribution and drug resistance of clinical bacterial isolated from common infections in a hospital from 2018 to 2020,so as to provide a referential basis for clinicians to enhance rational use of antibiotics.Methods The distribution and drug resistance data of bacterial isolated from the hospital clinical specimens from 2018 to 2020 were retrospectively analyzed,and the Antimicrobial Susceptibility Test results were interpreted based on the file issued by the American Society for clinical and laboratory standardization in 2019.Results From 2018 to 2020,a total of 6 862 bacteria were detected,of which 840 were gram-positive bacteria(12.24%),5 863 were gram-negative bacteria(85.44%).The results showed negative for Vancomycin and linezolid resistant Staphylococcus.However,only a few results were positive for Vancomycin resistant Enterococcus.The top four detection rates of gram-negative bacteria were klebsiella pneumoniae [19.67%(1 350/6 862)],Singular deformed rod [8.96%(615/6 862)],Escherichia coli [7.74%(531/6 862)],Serratia marcescens[2.93%(201/6 862)].Enterobacteriaceae had low resistance rates to β-lactams,aminoglycosides and carbapenems,and A.baumannii had a high resistance rate to imipenem(77.01%).Conclusion The analysis of bacterial resistance is of great significance to the rational application and scientific management of antimicrobial agents in hospitals,therefore,surveillance of bacterial resistance should be strengthened.
