抑郁症合并阻塞性睡眠呼吸暂停患者的记忆损伤指标及相关性

Memory impairment indexes in patients with depression complicated with obstructive sleep apnea and its correlation analysis

目的 探讨抑郁症合并阻塞性睡眠呼吸暂停(OSA)患者的记忆损伤指标及相关性。方法 109例抑郁症患者,根据24 h多导睡眠图(PSG)监测结果分为伴OSA抑郁症组(n=43,伴有OSA)和单纯抑郁症组(n=66);比较2组患者的汉密尔顿抑郁量表(HAMD)、抑郁自评量表(SDS)评分、睡眠呼吸参数氧减指数、快速动眼睡眠时间百分比(REM%)、血氧饱和度<90%占监测时间的百分比(SaT90)、前瞻性记忆指标基于事件前瞻性记忆(EBPM)、基于时间前瞻性记忆(TBPM)评分,采用简单线性相关分析法分析伴OSA抑郁症组患者EBPM评分、TBPM评分与睡眠参数的相关性,EBPM、TBPM评分与HAMD、SDS评分的相关性。结果 伴OSA抑郁症组患者的HAMD评分、SDS评分及AHI、氧减指数、SaT90值均高于单纯抑郁症组,EBPM评分、TBPM评分、REM%均低于单纯抑郁症组,差异具有统计学意义(P<0.05);伴OSA抑郁症组患者的REM(%)与EBPM评分、TBPM评分无显著的相关关系(P> 0.05);简单线性相关分析结果显示,伴OSA组的AHI、氧减指数、SaT90值于EBPM评分、TBPM评分均呈显著的负相关关系(P<0.05),EBPM评分、TBPM评分与HAMD评分、SDS评分无显著的相关性(P> 0.05)。结论 抑郁症合并OSA患者出现明显的前瞻性记忆损伤较单纯抑郁症患者严重,记忆损伤程度与OSA病情程度有一定的相关性。

Objective To investigate the memory impairment indexes and their correlations in patients with depression complicated with obstructive sleep apnea(OSA).Methods One and nine patients with depression were divided into depression with OSA group(n=43,with OSA)and simple depression group(n=66)according to the monitoring results of 24-hour polysomnography(PSG).Hamilton Depression Scale(HAMD),Self-rating Depression Scale(SDS)score,sleep breathing parameters oxygen reduction index,percentage of rapid eye movement sleep time(REM%),percentage of blood oxygen saturation<90%in total monitoring time(SaT90),prospective memory index[event-based prospective memory(EBPM)],time-based prospective memory(TBPM)score;simple linear correlation analysis was used to analyze the EBPM score,TBPM score,and sleep parameters of depression with OSA group;the correlation of EBPM,TBPM scores with HAMD and SDS scores.Results The HAMD score,SDS score,AHI,hypoxia index,and SaT90 value of patients with OSA depression group were higher than those of simple depression group;and the EBPM score,TBPM score,and REM%were lower than those of simple depression group,and the differences were statistically significant(P<0.05).There was no significant correlation between REM(%)and EBPM score/TBPM score in the depression with OSA group(P>0.05);the results of simple linear correlation analysis showed that AHI,hypoxia index,SaT90 values of OSA group were significantly negatively correlated with EBPM score and TBPM score(P<0.05),while EBPM score and TBPM score had no significant correlation with HAMD score and SDS score(P>0.05).Conclusion Depression patients with OSA have obvious prospective memory impairment than those with simple depression,and the degree of memory impairment has a certain correlation with the severity of OSA.
