小儿支气管哮喘发作期血清miR-146a、miR-21及miR-126水平与哮喘严重程度的相关性 被引量:2

摘要 目的 探讨小儿支气管哮喘发作期血清微小RNA-146a(miR-146a)、微小RNA-21(miR-21)及微小RNA-126(miR-126)水平与哮喘严重程度的相关性。方法 选取130例哮喘发作期患儿作为观察组、再根据患儿哮喘严重程度分为轻度组(n=41)、中度组(n=56)及重度组(n=33),选取同期130例体检正常儿童作为对照组;采用实时荧光定量(qRT-PCR)法检测受试儿童血清miR-146a、miR-21及miR-126表达水平,比较对照组与观察组、不同哮喘程度3组患儿的血清miR-146a、miR-21及miR-126水平;采用Spearman秩相关分析血清miR-146a、miR-21及miR-126水平与患儿哮喘严重程度的相关性,采用受试者工作曲线(ROC)下面积(AUC)分析miR-146a、miR-21、miR-126对小儿支气管哮喘发作的预测价值。结果 观察组患儿血清miR-146a、miR-21、miR-126表达水平高于对照组儿童,差异有统计学意义(P<0.001);3组不同严重程度哮喘患儿的血清miR-146a、miR-21、miR-126表达水平比较,轻度组<中度组<重度组,差异有统计学意义(P<0.05);Spearman秩相关分析结果显示,血清miR-146a、miR-21、miR-126表达水平与哮喘患儿的病情严重程度呈正相关(P<0.05);经ROC分析发现血清miR-146a、miR-21、miR-126水平的AUC为0.738、0.767、0.869(P<0.05),均可用于小儿支气管哮喘发作的预测。结论 小儿支气管哮喘发作期患儿血清miR-146a、miR-21、miR-126均呈高表达,其水平与支气管哮喘严重程度呈正相关,3项指标检测对支气管哮喘发作具有一定的预测价值。 Objective To investigate the correlation between serum levels of microRNA-146a(miR-146a),microRNA-21(miR-21),and microRNA-126(miR-126)during bronchial asthma attack and the severity of asthma in children.Methods A total of 130 children with asthma attack were selected as the observation group,and then divided into mild group(n=41),moderate group(n=56),and severe group(n=33)according to the severity of asthma.One hundred and thirty children receiving normal physical check were selected as the control group.The expression levels of miR-146a,miR-21,and miR-126 in peripheral blood of subjects were detected by qRT-PCR.The serum levels of miR-146a,miR-21,and miR-126 in control group,observation groups/three groups with different severity of asthma were compared.Spearman rank correlation analysis was used to analyze the correlation between serum miR-146a,miR-21,and miR-126 levels and the severity of asthma in children.The area under the receiver operating characteristic(ROC)curve(AUC)was used to analyze the predictive value of miR-146a,miR-21,and miR-126 on bronchial asthma attacks in children.Results The expression levels of serum miR-146a,miR-21,and miR-126 in observation group were higher than those of control group,and the differences were statistically significant(P<0.001).The expression levels of miR-146a,miR-21,and miR-126 in serum of asthmatic children with different severity in three groups were compared:mild group<moderate group<severe group,and the differences were statistically significant(P<0.05).Spearman rank correlation analysis showed that the expression levels of serum miR-146a,miR-21,and miR-126 were positively correlated with the severity of asthma in children(P<0.05).ROC analysis showed that the AUC of serum level of miR-146a,miR-21,and miR-126 were 0.738,0.767,and 0.869(P<0.05),which could be used to predict the onset of bronchial asthma in children.Conclusion Serum miR-146a,miR-21,and miR-126 in children with bronchial asthma attack are all highly expressed,and they are positively correlated with severity of bronchial asthma.The three indicators test show certain predictive value for bronchial asthma attack.
