消渴康肾方辅助治疗糖尿病肾病临床研究

Clinical Study on Xiaoke Kangshen Prescription in Adjuvant Treatment of Diabetic Nephropathy

目的:观察消渴康肾方治疗气阴两虚兼瘀血内阻型糖尿病肾病的临床疗效。方法:采用随机数字表法将106例气阴两虚兼瘀血内阻型糖尿病肾病患者分成对照组与观察组各53例。对照组给予常规西药治疗,观察组在对照组的基础上加用消渴康肾方治疗,2组连续治疗3个月。比较2组治疗前后中医症状评分、血糖指标、肾功能指标,并评价2组临床疗效及不良反应发生情况。结果:观察组总有效率为94.12%,高于对照组76.47%(P<0.05),观察组临床疗效优于对照组(P<0.05)。治疗后,2组主症、次症、舌脉中医症状评分及总分均较治疗前降低(P<0.05),且观察组降低幅度大于对照组(P<0.05);2组空腹血糖(FBG)、餐后2 h血糖(P2hBG)、糖化血红蛋白(HbA1c)水平均较治疗前降低(P<0.05),观察组治疗后血糖指标低于对照组(P<0.05);2组24 h尿蛋白定量、尿素氮、血肌酐水平较治疗前降低(P<0.05),肾小球滤过率较治疗前升高(P<0.05),观察组治疗后上述指标改善优于对照组(P<0.05)。2组不良反应发生率比较,差异无统计学意义(P>0.05)。结论:消渴康肾方辅助治疗气阴两虚兼瘀血内阻型糖尿病肾病,能降低患者血糖水平,改善肾功能,提高临床疗效,安全性较高。

Objective:To observe the clinical effect of Xiaoke Kangshen prescription for diabetic nephropathy of dual deficiency of qi and yin complicated with internal obstruction of static blood type.Methods:A total of 106 cases of patients with diabetic nephropathy of dual deficiency of qi and yin complicated with internal obstruction of static blood type were divided into the control group and the observation group according to the random number table method,with 53 cases in each group.The control group was treated with routine western medicine,and the observation group was additionally treated with Xiaoke Kangshen prescription based on the treatment of the control group.Both groups were treated for three months.Chinese medicine symptom scores,blood glucose indexes and kidney function indexes before and after treatment in the two groups were compared;the clinical effects and the incidence of adverse reactions in the two groups were evaluated.Results:The total effective rate was 94.12%in the observation group,higher than that of 76.47%in the control group(P<0.05),and the clinical effect in the observation group was better than that in the control group(P<0.05).After treatment,the scores of primary symptoms,secondary symptoms and Chinese medicine symptoms of tongue and pulse as well as the total scores in the two groups were decreased when compared with those before treatment(P<0.05),and the decrease in the observation group was more significant than that in the control group(P<0.05);the levels of fasting blood glucose(FBG),two-hour postprandial blood glucose(P2hBG)and glycosylated hemoglobin(HbA1c)in the two groups were decreased when compared with those before treatment(P<0.05),and the blood glucose indexes in the observation group were lower than those in the control group(P<0.05).The levels of 24-hour urinary protein quantification,urea nitrogen and serum creatinine in the two groups were decreased when compared with those before treatment(P<0.05),and the glomerular filtration rates were increased(P<0.05);the improvement of the above indexes in the observation group was better than that in the control group(P<0.05).There was no significant difference being found in the comparison of the incidence of adverse reactions between the two groups(P>0.05).Conclusion:Xiaoke Kangshen prescription for diabetic nephropathy of dual deficiency of qi and yin complicated with internal obstruction of static blood type can reduce blood glucose levels,improve kidney function and enhance clinical effect with high safety.
