积极心理学视角下造血干细胞移植术后患者癌症复发恐惧现状及其影响因素研究

The fear of cancer recurrence and its influencing factors in patients after Hematopoietic Stem Cell Transplantation from the perspective of positive psychology

目的:基于积极心理学分析造血干细胞移植术后患者癌症复发恐惧现状及其影响因素。方法:选取天津市某三级甲等医院2021年5月—8月住院及复查的造血干细胞移植术后患者226名,采用一般资料调查问卷、恐惧疾病进展简化量表、简易疾病感知问卷、Herth希望指数、家庭弹性评定量表中文简化版、领悟社会支持量表进行调查。结果:36.7%的造血干细胞移植术后患者存在高水平癌症复发恐惧;造血干细胞移植术后患者癌症复发恐惧得分为(31.14±9.29)分;多元逐步回归分析显示,医疗负担、疾病感知、领悟社会支持是患者癌症复发恐惧的影响因素,解释总变异量的32.5%。结论:造血干细胞移植术后患者的癌症复发恐惧问题较为严重,医护人员应重视患者的心理状态,采取相应的干预措施,鼓励患者以积极的态度面对疾病,提高患者对疾病的正确认知及领悟社会支持能力,从而降低患者的癌症复发恐惧水平。

Objective:To explore the fear of cancer recurrence and its influencing factors in patients after Hematopoietic Stem Cell Transplantation(HSCT)from the perspective of positive psychology.Methods:From May to August 2021,a total of 226 hospitalized and reexamined patients after HSCT were investigated with general information questionnaire,the Fear of Progression Questionnaire-Short Form(FoP-Q-SF),the Brief Illness Perception Questionnaire(BIPQ),the Herth Hope Index(HHI),the Shortened Chinese Version of the Family Resilience Assessment Scale(FRAS-C),and the Perceived Social Support Scale(PSSS).Results:36.7%of patients after HSCT have a high level of fear of cancer recurrence.The mean score of FoP-Q-SF after HSCT was 31.14±9.29.Multiple stepwise regression analysis showed that medical burden,illness perception,perceived social support were the main influencing factors of fear of cancer recurrence,which explained 32.5%of the total variance.Conclusion:The fear of cancer recurrence in patients after HSCT is a serious problem.It is suggested that medical staff should pay attention to the psychological state of patients,take measures to encourage patients to face the disease with a positive attitude,improve patients'correct cognition of the disease and perceived social support,so as to reduce patients'fear of cancer recurrence.
