子宫腺肌病患者的中医体质分布特点

Distribution of Traditional Chinese Medicine Constitution in Patients with Adenomyosis

【目的】初步探讨子宫腺肌病患者中医体质分布特点,为临床干预提供客观依据。【方法】采用《中医体质分类与判定表》收集323例子宫腺肌病患者的一般资料和体质类型,对不同年龄阶段、不同病程、有无痛经的患者中医体质类型的分布进行统计学分析。【结果】(1)子宫腺肌病患者的体质存在多种偏颇体质的兼夹,同时存在2种以上的体质类型占比达79.57%。(2)323例子宫腺肌病患者的中医体质中,以阳虚质所占比例最大,为137例次(42.41%),其他中医体质分布频率从高到低依次为血瘀质>气郁质>平和质>气虚质>湿热质>痰湿质=阴虚质>特禀质。(3)平和质在不同病程、不同年龄阶段、有无痛经患者中的构成比不同,差异均有统计学意义(P<0.05);在有痛经及病程1年以上的子宫腺肌病患者中阳虚质所占比例更高,差异均有统计学意义(P<0.05)。【结论】阳虚质、血瘀质为子宫腺肌病患者的主要体质类型,从阳虚、血瘀角度调理子宫腺肌病具有一定优势。

Objective To preliminarily explore the distribution of traditional Chinese medicine(TCM)constitution in the patients with adenomyosis(AM)and to provide objective evidence for its clinical intervention.Methods The general information and constitution types of 323 patients with AM were collected using the Traditional Chinese Medicine Constitution Classification and Distinguishing Scale,and the distribution of TCM constitution types in patients with various age groups,course of disease and presence or absence of dysmenorrhea was statistically analyzed.Results(1)Patients with AM had the complication of a variety of biased constitution types,and 79.57%of them had the complicated constitution with more than 2 types of constitution types.(2)Among the 323 patients with AM,most of the patients had yang deficiency constitution,which accounted for the largest proportion of42.41%with 137 cases.The other TCM constitution types were distributed in descending order of frequency as blood stasis constitution>qi stagnation constitution>balanced constitution>qi deficiency constitution>dampheat constitution>phlegm damp constitution=yin deficiency constitution>inherited special constitution.(3)The proportion of balanced constitution varied among the patients with various course of disease,age groups and presence or absence of dysmenorrhea,and the differences were all statistically significant(P<0.05).The AM patients with dysmenorrhea and with more than one year of the disease had higher proportion of yang deficiency constitution,and the differences were all statistically significant(P<0.05).Conclusion Yang deficiency constitution and blood stasis constitution are the primary constitution types in adenomyosis patients.Intervention of adenomyosis from the perspective of yang deficiency and blood stasis has certain advantages.
