医联体医疗资源配置政策文本量化分析与建议

Text quantitative analysis and suggestions on medical resource allocation policy of medical alliance

目的对医联体医疗资源相关政策进行政策文本分析,解析现状、剖析问题、提出措施。方法以医联体或者医疗联合体为关键词,检索2015-2020年在国务院办公厅、国家卫生健康委等机关部门官方网站相关政策文本,采取政策文本量化法,分别从政策关键词、政策工具角度对医联体医疗资源相关政策文本进行量化分析与评价。结果不同政策工具的使用比例差异明显:环境型政策工具使用较多;需求型政策工具使用较少,在加强人力、物力和财力三方联动政策机制的情况下应进一步整合资源共享机制,并完善资源配置的指标标准;存量配置政策工具使用较少,尤其是现有医务人员资源的优化配置方面。结论政府部门应加强关于绩效考核、薪酬待遇方面的政策的引导;此外,国家与地方政策相结合,进一步加强和完善医疗保险支付方式。

Objective To conduct policy text analysis on the medical resources related policies of the medical alliance to explain the current situation,dissect the problems,and put forward measures.Method The relevant policy texts from the official websites of the General Office of the State Council,the National Health Commission and other agencies and departments from 2015 to 2020 were retrieved with the keywords of the medical alliance.The quantitative method of policy text was adopted to quantitatively analyze and evaluate the relevant policy texts of the medical resources of the medical alliance from the perspectives of policy keywords and policy instruments,respectively.Results There were differences in use proportion of policy instruments:The environmental policy instruments were more used;demand-oriented policy instruments were less used.Based on strengthening the threeparty linkage policy mechanism of human,material and financial resources,the resource sharing mechanism should be further integrated and the standards of resource allocation should be improved.Stock allocation policy instruments were less used,especially in the optimal allocation of existing medical staff resources.Conclusion The government should strengthen the guidance of policies on performance appraisal and salary;In addition,national and local policies should be combined to further strengthen and improve the payment methods of medical insurance.
