药学监护是为了改善患者的生存质量而由临床药师直接、负责任地向患者及其家属和医务人员等提供药物使用有关的服务。本文探讨临床药师针对1例精神分裂症合并糖尿病患者开展的药学监护,以病人为中心,临床药师从诊疗经过、血糖监护、不良反应监护、用药教育、生活指导等方面,结合自身药学优势,协助临床医师优化治疗方案,提高治疗的有效性和安全性,达到实现个体化治疗的目的,为精神分裂症合并糖尿病患者的药学监护提供参考依据。

In order to improve the quality of life of patients, pharmaceutical care is provided by clinical pharmacists directly and responsibly to patients, their families and medical staff. This article explores the clinical pharmacists in 1 case of schizophrenia patients with diabetes mellitus in pharmaceutical care, patient centered, clinical pharmacists through diagnosis, blood sugar monitoring, adverse reaction monitoring, drug education, life guidance, etc., combining with advantages of own pharmacy, to assist clinicians to optimize treatment, improve the efficacy and safety of the treatment, to achieve the purpose of individualized treatment, to provide a reference for the pharmaceutical care of schizophrenia patients with diabetes.
