浮针治疗原发性面肌痉挛的临床观察

Clinical Observation of Floating Needling in the Treatment of Primary Hemifacial Spasm

【目的】观察浮针治疗原发性面肌痉挛的临床疗效。【方法】将60例面肌痉挛患者随机分为对照组和观察组,每组各30例。对照组给予常规针刺治疗,观察组给予浮针治疗,均连续治疗2周。治疗2周后,评价2组临床疗效,观察2组患者治疗前后面肌痉挛强度与频度分级的变化情况,以及面神经F波运动潜伏期、波幅及面神经传导速度的情况。比较2组患者治疗前后生存质量测定量表(WHOQOL-BREF)评分的变化情况。【结果】(1)治疗后,2组患者的面肌痉挛强度分级与面肌痉挛频度分级均明显改善(P<0.05),且观察组的改善程度明显优于对照组,差异有统计学意义(P<0.05)。(2)治疗后,2组患者的F波时相及波幅均明显下降(P<0.05);且观察组在改善F波时相及波幅方面明显优于对照组,差异均有统计学意义(P<0.05)。(3)治疗后,2组患者的健侧面神经传导速度比较,差异无统计学意义(P>0.05),2组患者的患侧面神经传导速度均显著升高(P<0.05);且观察组在改善患侧面神经传导速度方面明显优于对照组,差异有统计学意义(P<0.05)。(4)治疗后,2组患者WHOQOL-BREF评分在生理、心理、社会关系及环境各维度评分方面均明显提升,差异有统计学意义(P<0.05),且观察组在改善WHOQOL-BREF评分方面明显优于对照组,差异有统计学意义(P<0.05)。(5)观察组总有效率为80.00%(24/30),对照组为70.00%(21/30)。观察组疗效优于对照组,差异有统计学意义(P<0.05)。【结论】浮针治疗原发性面肌痉挛,能有效缓解患者的临床症状,改善患侧面神经传导速度,从而提高患者生活质量,疗效显著。

Objective To observe the clinical efficacy of floating needling in the treatment of primary hemifacial spasm.Methods Sixty patients with hemifacial spasm were randomly divided into the control group and the observation group,with 30 cases in each group.The control group was given conventional acupuncture treatment and the observation group was given floating needling treatment.The treatment in both groups lasted 2 weeks.After2 weeks of treatment,the clinical efficacy of the two groups was evaluated.The changes in the intensity grading and frequency grading of posterior myospasm,as well as the latency of facial nerve F-wave movement,wave amplitude and facial nerve conduction velocity were observed in the two groups before and after treatment.The changes in the World Health Organization Quality of Life(WHOQOL-BREF)scores in the two groups were compared before and after treatment.Results(1)After treatment,the intensity grading and the frequency grading of hemifacial spasm in both groups were significantly improved(P<0.05),and the grading of improvement in the observation group was significantly superior to that in the control group,the difference being statistically significant(P<0.05).(2)After treatment,the F-wave temporal phase and wave amplitude of both groups were significantly decreased(P<0.05),and the observation group was significantly superior to the control group in improving the Fwave temporal phase and wave amplitude,the difference being statistically significant(P<0.05).(3)After treatment,there was no statistically significant difference in the conduction velocity of the acute lateral nerve between the two groups(P>0.05),while the conduction velocity of the affected lateral nerve was significantly increased in both groups(P<0.05),and the observation group was significantly superior to the control group in improving the conduction velocity of the affected lateral nerve,the differences being statistically significant(P<0.05).(4)After treatment,the WHOQOL-BREF scores of patients in the two groups were significantly improved in all dimensions of physical,psychological,social relationship and environmental scores,the difference being statistically significant(P<0.05),and the observation group was significantly superior to the control group in improving the WHOQOL-BREF scores,the difference being statistically significant(P<0.05).(5)The total effective rate was 80.00%(24/30)in the observation group and 70.00%(21/30)in the control group.The efficacy of the observation group was superior to that of the control group,and the difference was statistically significant(P<0.05).Conclusion Floating needling is effective in treating primary hemifacial spasm by relieving patients’clinical symptoms and improving the conduction velocity of the affected facial nerves,thereby improving their quality of life with remarkable efficacy.
