经阴道彩色多普勒超声在药物流产后宫内残留物患者诊断的应用价值

Application Value of Transvaginal Color Doppler Ultrasound in the Diagnosis of Patients with Intrauterine Residue after Drug Abortion

目的:研讨药物流产后宫内残留物患者接受经阴道彩色多普勒超声(TVC)检测的临床价值。方法:选取2018年9月—2021年1月入住我院行药物流产且术后持续2周以上伴阴道不规则出血的患者80例,对所选患者行TVC、经腹部彩色多普勒超声(TAC)诊断,并结合其病理检查资料,评估TVC的诊断准确情况。结果:80例药物流产患者中,有74例经病理检查证实存在宫内残留物。其中TVC对患者宫内残留物的诊断准确度为96.25%(77/80),高于TAC的准确度73.75%(59/80),差异有统计学意义(P<0.05)。宫内残留物血流信号检测显示,TVC提示有彩色血流信号显示者占比为95.95%(71/74),高于TAC检测结果77.03%(57/74),比较有统计学意义(P<0.05)。结论:药物流产后使用TVC诊断,能够更准确判断患者有无宫内组织物残留,进而为后续治疗提供更加客观、准确的参考数据。

Objective:To investigate the clinical value of transvaginal color Doppler ultrasound(TVC)in detecting intrauterine residues after drug abortion.Method:Select 80 patients who were admitted to our hospital from September 2018 to January 2021 for medical abortion and continued for more than 2 weeks after operation with irregular vaginal bleeding.The selected patients were diagnosed by TVC and transabdominal color Doppler ultrasound (TAC). Combined with their pathological examination data, the diagnostic accuracy of TVC was evaluated. Result: Among 80 cases of medical abortion, 74 cases were confirmed to have intrauterine residues by pathological examination. The diagnostic accuracy of TVC for intrauterine residues was 96.25% (77/80), which was higher than that of TAC 73.75% (59/80), and the difference was statistically significant ( P <0.05). The blood flow signal detection of intrauterine residues showed that the proportion of TVC indicating color blood flow signal was 95.95% (71/74), which was higher than the TAC detection result of 77.03% (57/74), and the comparison was statistically significant ( P <0.05). Conclusion: TVC diagnosis after drug abortion can more accurately judge whether patients have intrauterine tissue residues, and then provide more objective and accurate reference data for follow-up treatment.
