"4+7"带量采购政策下某院门诊冠心病患者用药情况分析

Drug use of outpatients with coronary heart disease in a hospital under "4+ 7" procurement with quantity policy

目的分析"4+7"带量采购政策下新乡市康复医院门诊冠心病患者药物使用情况,为临床药学服务和医院药事管理提供参考。方法采用回顾性分析方法,收集医院自执行"4+7"带量采购前1年及之后2年(2019年1月1日至2021年12月31日)门诊冠心病处方数据,对患者性别、年龄,用药种类、消耗量、用药频度(DDDs)和日均治疗费用(DDC)等进行统计分析。结果冠心病患者男女比例为2∶1,年龄60~69岁者占比由2019年的19.10%升至2021年的27.09%,为患病主体人群;"4+7"集采后,使用频度高的药品中有7种已执行"4+7"带量采购政策,DDC明显下降,其中瑞舒伐他汀钙片、阿托伐他汀钙片、厄贝沙坦片DDC值分别下降95.23%、93.06%、90.44%。结论"4+7"带量采购政策降低了患者用药的日均费用,切实减轻了冠心病患者的用药负担。

Objective To analyze the drug use of outpatients with coronary heart disease(CHD)in Xinxiang Rehabilitation Hospital under"4+7"procurement with quantity policy,and to provide reference for clinical pharmacy services and hospital pharmacy management.Methods The retrospective analysis method was used in the study,and the prescription data from CHD outpatient in the hospital from 1 year before"4+7"procurement with quantity to 2 years after"4+7"procurement with quantity(from January 1st,2019 to December 31st,2021)were collected,and the gender,age,drug type,consumption,defined daily dose system(DDDs)and defined daily dose cost(DDC)of patients were statistically analyzed.Results The proportion of male to female patients with CHD was 2∶1,and the proportion of patients aged 60-69 years increased from 19.10%in 2019 to 27.09%in 2021,which was the main population of CHD.After the implementation of"4+7"policy on volume procurement,7 kinds of drugs with high frequency of use had implemented the"4+7"policy on volume procurement,and the DDC decreased significantly.The DDC values of rosuvastatin calcium tablets,atorvastatin calcium tablets and irbesartan tablets decreased by 95.23%,93.06%and 90.44%,respectively.Conclusions The"4+7"purchase policy with quantity reduced the daily cost of medication and effectively reduced the medication burden of patients with CHD.
