扶正解毒消积方对原发性肝癌患者肝动脉化疗栓塞术后肝功能及不良反应的影响

Effect of Fuzheng Jiedu Xiaoji Formula on liver function and adverse reactions in patients with primary liver cancer after transcatheter arterial chemoembolization

目的观察扶正解毒消积方对原发性肝癌患者肝动脉化疗栓塞术后肝功能及不良反应的影响。方法将120例原发性肝癌患者按照随机数字表法分为2组,对照组64例予肝动脉化疗栓塞术,治疗组56例在肝动脉化疗栓塞术后应用扶正解毒消积方,3个月为1个疗程,治疗3个疗程。比较2组治疗前及疗程结束后3、7 d肝功能指标血清丙氨酸氨基转移酶(ALT)、总胆红素(TBiL)、白蛋白(ALB)、γ-谷氨酰转移酶(GGT)水平变化;观察2组第1次肝动脉化疗栓塞术后不良反应发热、呕吐、疼痛发生情况;观察2组患者肿瘤无进展生存期。结果治疗组疗程结束后3 d ALT、TBiL水平均高于本组治疗前(P<0.05),疗程结束后7 d ALT水平高于本组治疗前(P<0.05);对照组疗程结束后3、7 d ALT、TBiL、GGT水平均高于本组治疗前(P<0.05);治疗组疗程结束后3、7 d ALT、TBiL、GGT水平均低于对照组(P<0.05);2组治疗前ALB水平与疗程结束后3、7 d比较差异均无统计学意义(P>0.05)。治疗组第1次肝动脉化疗栓塞术后发热、呕吐发生率低于对照组(P<0.05),2组第1次肝动脉化疗栓塞术后疼痛发生率比较差异无统计学意义(P>0.05)。治疗组肿瘤无进展生存期高于对照组(P<0.05)。结论扶正解毒消积方能改善原发性肝癌肝动脉化疗栓塞术后患者肝功能,减少对患者机体的毒性作用,减轻发热、呕吐、疼痛症状,改善生活质量,延长无进展生存期。

Objective To observe the effect of Fuzheng Jiedu Xiaoji Formula on liver function and adverse reactions in patients with primary liver cancer(PLC)after transcatheter arterial chemoembolization(TACE).Methods A total of 120 PLC patients were randomly assigned into the treatment group(n=56)and the control group(n=64).All patients were managed by TACE,and those in the treatment group were additionally treated with Fuzheng Jiedu Xiaoji Formula.The treatment continued for 3 courses(1 course for 3 months),the aim was to compare liver function indexes(alanine transaminase[ALT],total bilirubin[TBiL],albumin[ALB],gamma-glutamyltransferase[GGT])on days 3,7;first adverse reactions(fever,vomiting,pain)after TACE;progress-free survival(PFS).Results Compared with before treatment,ALT and TBiL on day 3 and ALT on day 7 in the treatment group were increased(all P<0.05);ALT,TBiL,GGT on days 3 and 7 in the control group were increased(P<0.05),which were decreased in the treatment group compared with the control group(P<0.05).The difference was not statistically significant in ALB in groups between day 0 and days 3,7(P>0.05).The incidence of first adverse reactions(fever,vomiting)after TACE was decreased(P<0.05)in the treatment group relevant to the control group.None of the incidence of first adverse reactions(pain)after TACE differed significantly between groups(P>0.05).PFS was higher in the treatment group than in the control group(P<0.05).Conclusion For PLC patients after TACE,Fuzheng Jiedu Xiaoji Formula can improve liver function,reduce toxic effects,relieve symptoms of fever,vomiting,pain,improve quality-of-life,and prolong PFS.
