膈下逐瘀汤加减治疗气滞血瘀型原发性肝癌近期疗效及对肝功能、血清肿瘤标志物的影响

Short-term effect of modified Gexia Zhuyu Decoction for primary liver cancer of Qi stagnancy and blood stasis type and how to impact on liver function and serum tumor markers

目的观察膈下逐瘀汤加减治疗气滞血瘀型原发性肝癌的近期疗效及对肝功能、血清肿瘤标志物的影响。方法将96例气滞血瘀型原发性肝癌患者通过奇偶数法均分为2组。对照组48例予FOLFOX4方案化疗,治疗组48例在对照组治疗基础上加膈下逐瘀汤加减治疗。比较2组近期疗效及治疗前后血清肿瘤标志物(甲胎蛋白、癌胚抗原)和肝功能指标[总胆红素(TBiL)、直接胆红素(DBiL)、丙氨酸氨基转移酶(ALT)、白蛋白(ALB)、γ-谷氨酰转移酶(GGT)、碱性磷酸酶(ALP)]水平。结果治疗组总缓解率56.25%(27/48),对照组总缓解率33.33%(16/48),治疗组总缓解率高于对照组(P<0.05)。治疗后2组血清甲胎蛋白及癌胚抗原水平均较本组治疗前降低(P<0.05),且治疗组均低于对照组(P<0.05)。治疗后,2组TBiL、DBiL、ALT、GGT、ALP水平均较本组治疗前下降(P<0.05),ALB水平均较本组治疗前升高(P<0.05),且治疗组TBiL、DBiL、ALT、GGT、ALP水平均低于对照组(P<0.05),ALB水平高于对照组(P<0.05)。结论膈下逐瘀汤加减治疗气滞血瘀型原发性肝癌近期疗效显著,可降低肿瘤标志物水平,改善肝功能。

Objective To observe the short-term effect of modified Gexia Zhuyu Decoction for primary liver cancer(PLC)of Qi stagnancy and blood stasis type and how to impact on liver function and serum tumor markers(TM).Methods The patients with PLC(Qi stagnancy and blood stasis type)were equally assigned into the treatment group(n=48)and the control group(n=48)by the odd and even number method;in addition to fluorouracil,leucovorin,and oxaliplatin(FOLFOX4)regimen,and the treatment group additionally received modified Gexia Zhuyu Decoction.The aim was to compare the short-term effect and the serum TM(alpha-fetoprotein[AFP],carcinoembryonic antigen[CEA]),liver function indexes(total bilirubin[TBiL],direct bilirubin[DBiL],alanine aminotransferase[ALT],albumin[ALB],glutamyltransferase[GGT],alkaline phosphatase[ALP]).Results The remission rate was 56.25%(27/48)in the treatment group,higher than that of 33.33%(16/48)in the control group(P<0.05).After treatment,the serum TM in groups was decreased(P<0.05),the treatment group was more prevalent(P<0.05);TBiL,DBiL,ALT,GGT,ALP in groups were decreased(P<0.05),while ALB in groups was increased(P<0.05);lower TBiL,DBiL,ALT,GGT,ALP levels,and higher ALB levels were detected in the treatment group than those in the control group(all P<0.05).Conclusion For patients with PLC(Qi stagnancy and blood stasis type),modified Gexia Zhuyu Decoction has significant short-term efficacy,can reduce TM levels and improve liver function.
