超声造影在卵巢良、恶性肿瘤鉴别诊断中的价值

The value of contrast-enhanced ultrasound in differential diagnosis of benign and malignant ovarian tumors

目的探讨超声造影在卵巢良、恶性肿瘤鉴别诊断中的临床价值。方法选取40例卵巢肿瘤患者,依据检查方法不同分为对照组与研究组,每组20例。对照组采用普通超声检查,研究组采用超声造影检查。比较两组诊断结果。结果与病理诊断比较,对照组卵巢良、恶性肿瘤诊断率差异有统计学意义(P<0.05),研究组卵巢良、恶性肿瘤诊断率比较,差异无统计学意义(P>0.05),研究组卵巢良、恶性肿瘤诊断率高于对照组,差异有统计学意义(P<0.05)。研究组卵巢恶性肿瘤患者血流阻力指数(RI)(0.43±0.17)低于卵巢良性肿瘤患者的(0.61±0.15),Alaczar评分(5.73±1.87)分高于卵巢良性肿瘤患者的(3.45±1.35)分,差异有统计学意义(P<0.05)。研究组卵巢恶性肿瘤患者增强峰值时间、开始增强时间及平均通过时间均短于良性肿瘤患者,增强曲线下面积及增强峰值速度均高于卵巢良性肿瘤患者,差异有统计学意义(P<0.05)。结论在卵巢良、恶性肿瘤鉴别诊断中,超声造影可显著提高肿瘤鉴别诊断准确率,具有显著应用价值。

Objective To explore the clinical value of contrast-enhanced ultrasound in the differential diagnosis of benign and malignant ovarian tumors.Methods 40 patients with ovarian tumors were divided into control group and study group according to different examination methods,with 20 cases in each group.The control group was examined by ordinary ultrasound,and the study group was examined by contrast-enhanced ultrasound.The diagnostic results of the two groups were compared.Results Compared with the pathological diagnosis,and control group in benign and malignant ovarian tumors diagnostic rate difference was statistically significant(P<0.05),the team benign and malignant ovarian tumors diagnostic rate comparison,there was no statistically significant difference(P>0.05),the team benign and malignant ovarian tumors diagnosis rate is higher than the control group,the difference was statistically significant(P<0.05).The blood flow resistance index(RI)of patients with ovarian malignant tumor(0.43±0.17)was lower than that of patients with ovarian benign tumor(0.61±0.15),and the Alaczar score(5.73±1.87)was higher than that of patients with ovarian benign tumor(3.45±1.35),the difference was statistically significant(P<0.05).In the study group,the peak enhancement time,start enhancement time and average passing time of patients with ovarian malignant tumor were shorter than those of patients with benign tumor,and the area under enhancement curve and peak enhancement speed were higher than those of patients with benign ovarian tumor,with statistical significance(P<0.05).Conclusion In the differential diagnosis of benign and malignant ovarian tumors,contrast-enhanced ultrasound can significantly improve the accuracy of tumor differential diagnosis,and has significant application value.
