自拟祛风养血润肤液药浴治疗血虚风燥夹瘀型尿毒症皮肤瘙痒的研究

Study on medical bath with self-made Qufeng Yangxue Runfu liquid on pruritus due to uremia with syndrome of blood deficiency,wind-dryness and blood stasis

目的观察自拟祛风养血润肤液药浴治疗血虚风燥夹瘀型尿毒症皮肤瘙痒的临床效果。方法将2020年1月-2021年1月河北省沧州中西医结合医院收治的80例血虚风燥夹瘀型尿毒症皮肤瘙痒患者按照随机数字表分为2组,对照组40例给予西医基础治疗,观察组40例在对照组基础上加用自拟祛风养血润肤液药浴治疗,2组疗程均为12周。记录2组干预前后改良Duo氏瘙痒评分,检测肾功能[血肌酐(Cr)、血尿素氮(BUN)]、血钙、血磷、全段甲状旁腺激素(iPTH)、超敏C反应蛋白(hs-CRP)、肿瘤坏死因子-α(TNF-α)和白细胞介素-6(IL-6)水平,统计2组临床疗效。结果与治疗前比较,治疗后2组改良Duo氏瘙痒评分中各单项评分及总评分均明显降低(P均<0.05),且治疗后观察组上述评分均明显低于对照组(P均<0.05)。与治疗前比较,治疗后2组Cr、BUN、血钙、血磷、iPTH、hs-CRP、TNF-α和IL-6水平均明显降低(P均<0.05),且治疗后观察组上述指标水平均明显低于对照组(P均<0.05)。观察组治疗总有效率为87.5%(35/40),明显高于对照组的65.0%(26/40),差异有统计学意义(P<0.05)。结论自拟祛风养血润肤液药浴治疗血虚风燥夹瘀型尿毒症皮肤瘙痒不仅能缓解皮肤瘙痒症状,改善肾功能,还能下调hs-CRP、TNF-α和IL-6水平,抑制机体微炎症状态。

Objective It is to observe the clinical effect of medical bath with self-made Qufeng Yangxue Runfu liquid in the treatment of pruritus due to uremia with syndrome of blood deficiency,wind-dryness and blood stasis.Methods Eighty patients with pruritus due to uremia with syndrome of blood deficiency,wind-dryness and blood stasis admitted to Cangzhou Hospital of Integrated Traditional Chinese and Western Medicine from January 2020 to January 2021 were divided into 2 groups according to the random number table.40 cases in the control group were treated with basic western medicine,and 40 cases in the observation group were treated with medicated bath with self-made Qufeng Yangxue Runfu liquid on the basis of the control group.Both groups were treated for 12 weeks.The modified Duo’s pruritus score was recorded before and after intervention in the two groups,and the levels of renal function indexes[serum creatinine(Cr),blood urea nitrogen(BUN)],serum calcium,serum phosphorus,intact parathyroid hormone(iPTH),high-sensitivity C-reactive protein(hs-CRP),tumor necrosis factor-α(TNF-α)and interleukin-6(IL-6)were detected,and the clinical efficacy of the two groups was calculated.Results Compared with those before treatment,each individual score and total score of the modified Duo's pruritus score in the two groups were significantly decreased after treatment(all P<0.05),and the above scores in the observation group were significantly lower than those in the control group(all P<0.05).Compared with before treatment,the levels of Cr,BUN,blood calcium,blood phosphorus,iPTH,hs-CRP,TNF-αand IL-6 in the two groups were significantly decreased after treatment(all P<0.05),and the levels of above indicators in the observation group after treatment were significantly lower than those in the control group(P<0.05).The total effective rate of the observation group was 87.5%(35/40),which was significantly higher than 65.0%(26/40)of the control group(P<0.05).Conclusion Medicated bath with Self-made Qufeng Yangxue Runfu liquid can not only relieve the symptoms of pruritus,improve renal function,but also down-regulate the levels of hs-CRP,TNF-αand IL-6,inhibit the micro-inflammatory state of the body in the treatment of pruritus due to uremia with syndrome of blood deficiency,wind-dryness and blood stasis.
