乳牙大面积龋患儿选择乳牙预成冠与树脂直接充填治疗方式的临床对照研究

Controlled clinical study of preformed crown and direct resin filling in children with extensive caries in primary teeth

目的对比乳牙预成冠与树脂直接充填治疗在乳牙大面积龋患儿中的应用价值。方法60例乳牙大面积龋患儿,经随机数字表法分为观察组与对照组,各30例。对照组接受树脂直接充填治疗,观察组接受乳牙预成冠治疗。对比两组临床疗效,修复效果,治疗后1、6个月的临床指标,并发症发生率。结果观察组总有效率为100.00%,高于对照组的80.00%,差异有统计学意义(P<0.05)。两组牙体完整性、继发龋、磨损情况对比,差异无统计学意义(P>0.05);观察组修复体情况、边缘密合度、邻接情况优于对照组,差异有统计学意义(P<0.05)。治疗后1、6个月,观察组咀嚼功能、发音功能、牙齿外观评分分别为(3.90±0.55)、(5.53±1.14)、(5.53±1.11)分与(7.33±1.15)、(7.50±1.22)、(7.36±1.18)分,均高于对照组的(3.23±0.77)、(4.30±1.12)、(4.73±1.14)分与(6.10±1.24)、(6.53±1.14)、(6.40±1.12)分,差异有统计学意义(P<0.05)。观察组并发症发生率为3.33%,低于对照组的26.67%,差异有统计学意义(P<0.05)。结论运用乳牙预成冠方法进行乳牙大面积龋修复效果较好,在牙齿外观、咀嚼功能、发音功能方面均优于树脂直接充填修复,具有临床推广意义。

Objective To compare the application value of preformed crown and direct resin filling in children with extensive caries in primary teeth.Methods A total of 60 cases of children with extensive caries in primary teeth were divided into observation group and control group by random numerical table,with 30 cases in each group.The control group was treated with direct resin filling,and the observation group was treated with preformed crown of primary teeth.The clinical efficacy,restoration effect,clinical indicators at 1 and 6 months after treatment,and the incidence of complications were compared between the two groups.Results The total effective rate in the observation group was 100.00%,which was higher than 80.00%in the control group,and the difference was statistically significant(P<0.05).There was no statistically significant difference in tooth integrity,secondary caries and wear between the two groups(P>0.05).The prosthesis condition,edge closeness and adjacency of the observation group were better than those of the control group,and the differences were statistically significant(P<0.05).At 1 and 6 months after treatment,the scores of masticatory function,pronunciation function and tooth appearance in the observation group were(3.90±0.55),(5.53±1.14),(5.53±1.11)points and(7.33±1.15),(7.50±1.22),(7.36±1.18)points,which were higher than(3.23±0.77),(4.30±1.12),(4.73±1.14)points and(6.10±1.24),(6.53±1.14),(6.40±1.12)points in the control group,and the differences were statistically significant(P<0.05).The incidence of complications in the observation group was 3.33%,which was lower than 26.67%in the control group,and the difference was statistically significant(P<0.05).Conclusion For the extensive caries restoration in primary teeth,the restoration effect of preformed crown is better than of resin direct filling restoration in the terms of tooth appearance,chewing function and pronunciation function,which contains clinical promotion significance.
