腹腔镜联合胆道镜手术治疗胆囊结石伴胆总管结石的效果研究

Study on effect of laparoscopic and endoscopic cooperative surgery in the treatment of cholecystolithiasis and choledocholithiasis

目的探析腹腔镜联合胆道镜手术治疗胆囊结石伴胆总管结石患者的临床效果。方法68例胆囊结石伴胆总管结石患者,根据治疗方案的不同分为比对组与测试组,各34例。比对组应用开腹手术方案治疗,测试组应用腹腔镜联合胆道镜手术方案治疗。比较两组患者的下床活动时间、肛门排气时间、在院治疗时间、治疗效果以及治疗前后的疼痛评分。结果测试组下床活动时间(1.50±0.24)h、肛门排气时间(33.51±4.21)h及在院治疗时间(11.35±1.21)d均短于比对组的(3.24±0.42)h、(85.65±6.25)h、(18.45±2.03)d,差异具有统计学意义(P<0.05)。测试组治疗总有效率97.06%高于比对组的73.53%,差异具有统计学意义(P<0.05)。治疗后,测试组疼痛评分(1.22±0.10)分低于比对组的(3.03±0.21)分,差异具有统计学意义(P<0.05)。结论腹腔镜联合胆道镜手术治疗胆囊结石伴胆总管结石患者的临床效果显著,可减轻患者机体疼痛程度,缩短康复时间。

Objective To discuss the clinical effect of laparoscopic and endoscopic cooperative surgery in the treatment of cholecystolithiasis and choledocholithiasis.Methods A total of 68 patients with cholecystolithiasis and choledocholithiasis were divided into comparison group and test group according to different treatment regimens,with 34 cases in each group.The comparison group was treated with laparotomy,and the test group was treated with laparoscopic and endoscopic cooperative surgery.The off-bed activity time,anal exhaust time,in-hospital treatment time,therapeutic effect,and pain score before and after treatment were compared between the two groups.Results The off-bed activity time(1.50±0.24)h,anal exhaust time(33.51±4.21)h and in-hospital treatment time(11.35±1.21)d in the test group were all shorter than(3.24±0.42)h,(85.65±6.25)h and(18.45±2.03)d in the comparison group,and the differences were statistically significant(P<0.05).The total effective rate 97.06%of the test group was higher than 73.53%of the comparison group,and the difference was statistically significant(P<0.05).After treatment,the pain score(1.22±0.10)points of the test group was lower than(3.03±0.21)points of the comparison group,and the difference was statistically significant(P<0.05).Conclusion Laparoscopic and endoscopic cooperative surgery has significant clinical effect on patients with cholecystolithiasis and choledocholithiasis,and can reduce the body pain and shorten the rehabilitation time.
