情志护理应用于老年骨折手术患者的临床效果及对睡眠质量的影响

Clinical Effect of Emotional Nursing on Sleep Quality of Elderly Patients Undergoing Fracture Surgery

目的:探究老年骨折手术采取情志护理的效果以及对患者应激效应、睡眠质量的影响。方法:选取2020年1月至2022年1月泉州市正骨医院收治的老年骨折手术患者64例作为研究对象,按照简单随机抽样法分为对照组和观察组,每组32例。对照组患者行常规护理,观察组患者在对照组护理基础上实行情志护理。比较2组患者的睡眠质量、生理应激反应等指标。结果:护理后,观察组患者匹兹堡睡眠质量指数(PSQI)评分、焦虑自评量表(SAS)和抑郁自评量表(SDS)评分及生理应激反应指标(血清皮质醇、肾上腺素及去甲肾上腺素)均比对照组低,2组比较差异均有统计学意义(均P<0.05);观察组满意度为100%,对照组满意度为84.38%,2组比较差异有统计学意义(P<0.05);观察组下肢深静脉血栓及术后感染率明显低于对照组,2组比较差异有统计学意义(P<0.05)。结论:老年骨折手术患者采取情志护理效果确切,可减轻或调节其生理、心理应激反应,改善睡眠质量,且减少术后不良事件发生率,进而提高患者的满意度,值得推广应用。

Objective:To explore the effect of emotional nursing in elderly fracture surgery and its related effects on patients’stress effect and sleep quality.Methods:A total of 64 elderly patients with fracture surgery who were admitted to Quanzhou Zhenggu Hospital from January 2020 to January 2022 were selected as the research objects.They were divided into two groups according to the simple sampling method,namely the control group(n=32)and the observation group(n=32).The cases included in the control group received basic nursing.On this basis,the cases included in the observation group received emotional nursing,and the relevant indicators were compared and analyzed,including sleep quality,physiological stress response,etc.Results:After nursing,Pittsburgh Sleep Quality Index(PSQI)in the observation group was lower than that in the control group(P<0.05),and the psychological stress response(anxiety and depression)and physiological stress response(serum cortisol,epinephrine and norepinephrine)in the observation group were lower than those in the control group(Ps<0.05).Satisfaction:100%in the observation group and 84.38%in the control group(P<0.05).Compared with the control group,the lower extremity deep venous thrombosis and infection rate in the observation group were significantly lower(P<0.05).Conclusion:Emotional nursing is effective in elderly fracture surgery,which can reduce or regulate the physiological and psychological stress effects of patients,improve sleep status,reduce the rate of postoperative adverse events,and have high patient satisfaction,which is worthy of promotion.
