Effects of Preoperative Uric Acid Level on the Wound Infection After Tophi Surgery

目的 探讨痛风石手术前不同程度的血尿酸(blood uric acid, BUA)水平对术后切口发生感染的影响。方法 将201例行痛风石手术的患者根据术前BUA水平分为高尿酸血症组(n=114)和正常尿酸组(n=87),比较术后两组患者切口感染发生率、术前准备天数、术后住院天数、住院费用等差异。采用Logistic回归分析研究痛风石手术后切口发生感染的危险因素。结果 高尿酸血症组术后切口感染发生率显著高于正常尿酸组(P<0.05),与正常尿酸组相比,高尿酸血症组术后住院天数明显延长、住院费用显著增加(P<0.05)。术前BUA水平增高是痛风石手术后切口发生感染的危险因素。结论 高尿酸血症是痛风石手术后切口发生感染的独立危险因素,术前应将BUA水平控制在正常范围。

Objective To investigate the effects of preoperative blood uric acid(BUA) on the risk of the wound infection after tophi surgery. Methods A total of 201 cases of tophi surgery were assigned into hyperuricemia group(n=114) and normal uric acid group(n=87) according to the preoperative BUA level. The differences in the incidence of wound infection, preoperative preparation days, postoperative hospitalization days and hospitalization expenses were compared between the two groups after operation. The risk factors of incision infection after tophi surgery were analyzed by Logistic regression. Results The incidence of postoperative wound infection in the hyperuricemia group was significantly higher than that in the normal uric acid group(P<0.05). Compared with normal uric acid group, the postoperative hospitalization days were significantly prolonged, and the hospitalization expenses significantly increased(P<0.05). Increased preoperative BUA level was a risk factor for wound infection after tophi surgery. Conclusion Hyperuricemia is an independent risk factor for wound infection after tophi surgery. The BUA level should be controlled within the normal range before tophi surgery.
