加巴喷丁联合脉冲射频治疗带状疱疹后遗神经痛患者的疗效及对血清炎症因子、睡眠、负性情绪的影响

Efficacy of gabapentin combined with pulsed radiofrequency in the treatment of postherpetic neuralgia patients and its effect on serum inflammatory factors, sleep and negative emotions

目的:探讨加巴喷丁联合脉冲射频(pulsed radiofrequency,PRF)治疗带状疱疹后遗神经痛(postherpetic neuralgia,PHN)患者的疗效及对血清炎症因子、睡眠、负性情绪的影响。方法:选择2018年10月至2021年10月邢台市中医院接诊的160例PHN患者作为研究对象,按照随机数表法分为观察组与对照组,每组80例。对照组使用加巴喷丁胶囊,观察组在对照组的基础上联合PRF治疗,两组均持续治疗2周。比较两组患者治疗前及治疗后1周、2周时视觉模拟量表(Visual Analogue Scale,VAS)评分、血清肿瘤坏死因子-α(tumor necrosis factorα,TNF-α)、白细胞介素-6(interleukin 6,IL-6)、阿森斯失眠量表(Athens Insomnia Scale,AIS)、广泛性焦虑自评量表(Generalized Anxiety Disorder Assessment,GAD-7)、患者健康问卷抑郁症候群量表(Patient Health Questionnaire,PHQ-9)的变化,并比较不良反应发生情况。结果:治疗后1周、2周时,观察组VAS评分、血清TNF-α、IL-6水平、AIS评分、GAD-7、PHQ-9评分结果均低于对照组,差异均有统计学意义(均P<0.05);两组治疗期间的不良反应发生率比较,差异无统计学意义(P>0.05)。结论:加巴喷丁联合PRF治疗PHN患者的疗效明显,可有效降低血清炎症因子的表达,积极改善患者睡眠质量及负性情绪,值得临床推广。

Objective:To study the efficacy of gabapentin combined with pulsed radiofrequency(PRF) in the treatment of postherpetic neuralgia(PHN) and its effects on serum inflammatory factors,sleep and negative emotions.Methods:A total of 160 PHN patients admitted to Xingtai Hospital of Traditional Chinese Medicine from October 2018 to October 2021 were selected as the study subjects,and were divided into an observation group and a control group according to random number table method,each group with 80 patients.The control group was treated with gabapentin capsule,and the observation group was treated with PRF on the basis of the control group.The treatment lasted for 2 weeks in both groups.The changes of Visual Analog Scale(VAS) score,serum tumor necrosis factor-α(TNF-α),interleukin-6(IL-6),Athens Insomnia Scale(AIS),Generalized Anxiety Disorder Assessment(GAD-7) and Patient Health Questionnaire(PHQ-9) before treatment and 1 and 2 weeks after treatment were compared between the 2 groups,and the incidence of adverse reactions were compared.Results:At after treatment 1 and 2 weeks,the VAS score,serum TNF-α and IL-6 levels,AIS score,GAD-7 and PHQ 9 scores in the observation group were lower than those in the control group,the difference were statistically significant(P<0.05);the incidence of adverse reactions were compared between the 2 groups,the difference was not statistically significant(P>0.05).Conclusion:Gabapentin combined with PRF have obvious efficacy in the treatment of PHN patients,which can effectively reduce the expression of serum inflammatory factors,and positively improve the sleep quality and negative emotions of patients,which is worthy of clinical promotion.
