血清HMGB1、APN水平对急诊脓毒症患者预后的影响

Influence of serum HMGB1 and APN levels on prognosis of emergency patients with sepsis

目的 探讨血清高迁移率族蛋白B1(HMGB1)、脂联素(APN)水平对急诊脓毒症患者预后的影响。方法 回顾性分析,收集2019年1月—2020年6月许昌市人民医院重症监护室收治的70例急诊脓毒症患者的临床资料,统计患者治疗28 d内病死(预后不良)及存活(预后良好)情况。收集患者基线资料,测定患者实验室指标,重点分析血清HMGB1、APN水平对急诊脓毒症患者预后的影响。结果 70例急诊脓毒症患者28 d病死20例,占28.57%;存活50例,占71.43%。预后不良组血清HMGB1水平高于预后良好组、血清APN水平低于预后良好组,差异有统计学意义(P<0.05);组间其他资料比较,差异无统计学意义(P>0.05)。经Cox生存回归分析结果显示,血清HMGB1水平升高、血清APN水平降低可能与急诊脓毒症患者治疗预后状况有关(P<0.05)。结论 血清HMGB1、APN水平影响急诊脓毒症患者预后情况,血清HMGB1水平升高、APN水平降低可增加急诊脓毒症患者预后不良风险。

Objective To investigate the influence of serum high mobility group B1(HMGB1) and adiponectin(APN) levels on the prognosis of emergency patients with sepsis.Methods A retrospective analysis was conducted, the clinical data of 70 patients with emergency sepsis admitted to the Intensive Care Unit of Xuchang People’s Hospital from January 2019 to June 2020 were collected, the mortality(poor prognosis) and survival(good prognosis) of patients within 28 days after treatment were counted.Baseline data of patients were collected, the laboratory indexes of patients were measured, the influence of serum HMGB1 and APN levels on the prognosis of emergency patients with sepsis were analyzed.Results Among 70 cases of patients with emergency sepsis, 20 cases died within 28 days, accounting for 28.57%;50 cases survived, accounting for 71.43%.The level of HMGB1 in poor prognosis group was higher than that in good prognosis group, and the level of APN was lower than that in good prognosis group, the differences were statistically significant(P<0.05);There was no statistical significant difference in other data between the two groups(P>0.05).By Cox regression analysis, the results showed that the increase of serum HMGB1 level and the decrease of serum APN level might be related to the prognosis of emergency patients with sepsis(P<0.05).Conclusion The serum levels of HMGB1 and APN can affect the prognosis of emergency patients with sepsis, increased serum HMGB1 level and decreased APN level can increase the risk of poor prognosis in emergency patients with sepsis.
