CT图像后处理技术鉴别肺部结节良恶性的价值 被引量:1

Value of CT image post-processing technology in differentiating benign and malignant pulmonary nodules

摘要 目的 分析CT图像后处理技术鉴别肺部结节良恶性的诊断价值。方法 选取2017年4月—2019年5月河南神火集团总医院收治的56例肺部结节患者作为研究对象。所有患者均接受CT图像后处理技术检查。以手术或穿刺活检病理检查结果为金标准,分析CT图像后处理技术诊断肺部结节良恶性的诊断效能,比较良、恶性结节血流信号分级情况。结果 56例肺部结节患者经手术或穿刺组织病理检查,32例(57.14%)确诊为良性结节,24例(42.86%)确诊为恶性结节,其中腺癌9例(16.07%),鳞癌15例(26.79%)。以手术或穿刺活检病理检查结果为“金标准”,CT图像后处理技术检出恶性肺部结节为25例(44.64%),良性结节31例(55.36%),CT图像后处理技术诊断肺部结节的灵敏度为91.67%(22/24),特异度为90.63%(29/32),准确度为91.07%(51/56);CT图像后处理技术诊断肺部结节良恶性与病理结果金标准K值为0.819,一致性好。恶性结节组血流信号分级0级、Ⅰ级、Ⅱ级、Ⅲ级分别为0例、2例(8.33)、10例(41.67)、12例(57.14),良性结节组血流信号分级0级、Ⅰ级、Ⅱ级、Ⅲ级分别为25例(78.13)、6例(18.75)、1例(3.13)、0例,恶性结节组血流信号分级高于良性结节组(P<0.05)。结论 CT图像后处理技术诊断肺部结节良恶性,具有较高诊断价值。 Objective To analyze the diagnostic value of CT image post-processing technology in differentiating benign and malignant pulmonary nodules.Methods Totally 56 patients with pulmonary nodules treated in Henan Shenhuo Group General Hospital from April 2017 to May 2019 were selected as the research subjects.All patients received CT image post-processing technology for examination.The results of surgical or biopsy and pathological examination were regarded as the gold standard, the diagnostic efficiency of CT image post-processing technology in the diagnosis of benign and malignant pulmonary nodules was analyzed, the blood flow signal grades of benign and malignant nodules were compared.Results Among 56 patients with pulmonary nodules through surgical or biopsy and pathological examination, 32(57.14%) were diagnosed as benign nodules and 24(42.86%) as malignant nodules, of which 9 cases were adenocarcinoma(16.07%) and 15 cases were squamous cell carcinoma(26.79%).The results of surgical or biopsy and pathological examination were regarded as the “gold standard”, there were 25 cases(44.64%) of malignant pulmonary nodules and 31 cases(55.36%) of benign pulmonary nodules detected by CT image post-processing technology, the sensitivity, specificity and accuracy of CT image post-processing technology in the diagnosis of pulmonary nodules were 91.67%(22/24), 90.63%(29/32) and 91.07%(51/56);The gold standard K value of CT image post-processing technology in the diagnosis of benign and malignant pulmonary nodules was 0.819, with a good consistency.In the malignant nodule group, there were 0 cases, 2 cases(8.33%), 10 cases(41.67%) and 12 cases(57.14%) with grade 0, grade Ⅰ, grade Ⅱ and grade Ⅲ of blood flow signal classification, respectively, in the benign nodule group, there were 25 cases(78.13%), 6 cases(18.75%), 1 case(3.13%) and 0 cases with grade 0, grade Ⅰ, grade Ⅱ and grade Ⅲ of blood flow signal classification, respectively, the Blood flow signal classification of blood flow signal in malignant nodule group was higher than that in benign nodule group(P<0.05).Conclusion CT image post-processing technology has high diagnostic value in the diagnosis of benign and malignant pulmonary nodules.
