椎旁肌退变与后路腰椎椎间融合术后引流量的关系

Association Between Paraspinal Muscles Degeneration and Drainage Volume After Posterior Lumbar Interbody Fusion

目的 探讨椎旁肌退变和后路腰椎椎间融合术(PLIF)后引流量的相关性,分析术后引流量产生的相关影响因素,从而对术后引流量进行预测并采取早期针对性措施。方法 回顾性筛选和分析2019年9月至2021年10月在巴彦淖尔市医院接受腰椎MRI检查及PLIF的105例住院患者的临床资料,利用Image J软件测量MRI中椎旁肌的脂肪浸润程度,使用SPSS 27.0统计软件对患者年龄、性别、民族、体质量指数(BMI)、手术节段数、合并高血压、合并糖尿病、术中出血量、椎旁肌脂肪浸润程度与PLIF术后引流量的关系进行统计分析。结果 单因素分析结果示不同年龄、术中失血量、合并糖尿病、手术节段数、多裂肌脂肪浸润程度患者的术后引流量比较,差异有统计学意义(P<0.05),年龄、术中失血量、手术节段数、多裂肌脂肪浸润程度与术后引流量呈正相关,性别、民族、BMI、合并高血压、竖脊肌脂肪浸润与术后引流量无相关性。多重线性回归方程示F=86.749,P<0.001,决定系数R~2=0.804,检验拟合度高,自变量中手术节段数、多裂肌脂肪浸润程度的相关系数高于其他。结论 PLIF术后引流量受多种因素影响,其中术前多裂肌退变的影像学参数与术后引流量呈正相关性。

Objective To investigate the association between paraspinal muscles degeneration and drainage volume after posterior lumbar interbody fusion(PLIF),and analyze the relevant influencing factors of postoperative drainage volume,so as to predict postoperative drainage volume and take early targeted measures.Methods The clinical data of 105 inpatients who underwent lumbar spine MRI and PLIF surgery in Bayannur Hospital from September 2019 to October 2021 were retrospectively screened and reviewed.Image J software was used to measure the fatty infiltration of paraspinal muscles in MRI,and SPSS 27.0 statistical software was used to analyze the association between age,gender,nationality,body mass index(BMI),number of operative segments,hypertension,diabetes,intraoperative blood loss,paraspinal muscle fatty infiltration and PLIF postoperative drainage volume.Results Univariate analysis showed that the postoperative drainage volume of patients with different age,intraoperative blood loss,diabetes mellitus,number of operative segments and fat infiltration degree of multifidus muscle were statistically difference(P<0.05).Age,intraoperative blood loss,number of operative segments and fat infiltration degree of multifidus muscle were positive with the postoperative drainage volume,and gender,nationality,BMI,hypertension and fat infiltration of erector spinae muscle had no correlation with the postoperative drainage volume.The multiple linear regression equation showed that F=86.749,P<0.001,the coefficient of determination R~2=0.804,the test fit was high,and the correlation coefficients of the number of operative segments and the degree of fatty infiltration of multifidus muscle among the independent variables were higher than others.Conclusion The postoperative drainage volume of PLIF is affected by many factors,among which the imaging parameters of multifidus degeneration are positively correlated with the postoperative drainage volume.
