摘要
目的调查渭南市城镇体检居民的糖代谢异常、高尿酸血症(HUA)、血脂异常及脂肪肝患病情况,同时研究糖代谢状态与血尿酸(BUA)、血脂、年龄、性别、脂肪肝之间的关系。方法选取2019年10月至2020年9月在渭南市中心医院健康管理中心进行体检的渭南市常住居民作为研究对象,分析人群构成特征及糖调节受损、糖尿病(DM)、HUA、血脂异常及脂肪肝患病率,同时分析糖代谢状态与BUA、血脂、年龄、性别、脂肪肝之间的关系。结果本研究共纳入10801名体检居民。共检出糖调节受损1207人,患病率为11.2%;共检出DM 647人,患病率为6.0%。男性的糖调节受损及DM患病率、空腹血糖(FBG)水平高于女性(P<0.05)。随着年龄增加,男性及女性的FBG水平及糖调节受损、DM患病率基本呈逐渐升高趋势。糖调节受损组及DM组的BUA、总胆固醇(TC)、甘油三酯(TG)、低密度脂蛋白胆固醇(LDL-C)水平明显高于正常血糖组,高密度脂蛋白胆固醇(HDL-C)水平明显低于正常血糖组(P<0.05)。DM组及糖调节受损组中脂代谢紊乱、脂肪肝患病率显著高于血糖正常组(P<0.05)。除高胆固醇血症及高LDL-C血症外,男性的其他各类脂代谢异常及HUA、脂肪肝患病率明显高于女性(P<0.05)。多因素有序Logistic回归分析结果显示,性别、年龄、BUA、TC、LDL-C、TG、脂肪肝均是糖代谢异常(糖调节受损及DM)发生的独立影响因素(P<0.05)。结论渭南市城镇居民的DM患病率明显低于全国患病率,男性和女性在糖代谢异常、HUA、脂代谢异常及脂肪肝患病率方面存在显著差异,糖代谢异常与BUA、脂代谢异常及脂肪肝密切相关。
Objective To investigate the prevalence of abnormal glucose metabolism,hyperuricemia(HUA),dyslipidemia and fatty liver among the urban residents with physical examination in Weinan city,and to study the relationship between glucose metabolism status and blood uric acid(BUA),blood lipids,age,gender and fatty liver.Methods The habitual residents of Weinan city who underwent physical examination in the health management center of Weinan Central Hospital from October 2019 to September 2020 were selected as the research objects.The characteristics of population composition and the prevalence rates of impaired glucose regulation,diabetes mellitus(DM),HUA,dyslipidemia and fatty liver were analyzed,and the relationship between glucose metabolism and BUA,blood lipids,age,gender and fatty liver was analyzed.Results A total of 10801 residents with physical examination were included in this study.A total of 1207 cases with impaired glucose regulation were detected,and a prevalence rate of 11.2%;a total of 647 cases with DM were detected,and a prevalence rate was 6.0%.The prevalence rates of impaired glucose regulation and DM,the level of fasting blood glucose(FBG)in males were higher than those in females(P<0.05).With the increase of age,the FBG level,the prevalence rates of impaired glucose regulation and DM in males and females basically showed gradually increasing trend.The levels of BUA,total cholesterol(TC),triglyceride(TG)and low density lipoprotein cholesterol(LDL-C)in the impaired glucose regulation group and the DM group were significantly higher than those in the normoglycemic group,and the level of high density lipoprotein cholesterol(HDL-C)was significantly lower than that in the normoglycemic group(P<0.05).The prevalence rates of lipid metabolism disorder and fatty liver in the DM group and the impaired glucose regulation group were significantly higher than those in the normoglycemic group(P<0.05).In addition to hypercholesterolemia and high LDL-C,the prevalence rates of abnormal lipid metabolism,HUA and fatty liver in males were significantly higher than those in females(P<0.05).The results of multivariate ordinal Logistic regression analysis showed that gender,age,BUA,TC,LDL-C,TG and fatty liver were all independent influencing factors of abnormal glucose metabolism(impaired glucose regulation and DM)(P<0.05).Conclusion The prevalence rate of DM in urban residents in Weinan city is significantly lower than that in whole country.There are significant differences in the prevalence rates of abnormal glucose metabolism,HUA,abnormal lipid metabolism and fatty liver between male and female.Abnormal glucose metabolism is closely related to BUA,abnormal lipid metabolism and fatty liver.
