尺骨鹰嘴截骨入路与内外侧入路非截骨入路内固定术治疗肱骨髁间骨折的效果比较

Comparison of effects of internal fixation via olecranon osteotomy approach and medial and lateral approach without osteotomy approach in the treatment of humeral intercondylar fractures

目的 比较尺骨鹰嘴截骨入路与内外侧入路非截骨入路内固定术治疗肱骨髁间骨折的效果。方法 选取2018年1月至2019年12月我院收治的78例肱骨髁间骨折患者作为研究对象,将其随机分为对照组和研究组,各39例。对照组接受尺骨鹰嘴截骨入路内固定术治疗,研究组接受内外侧入路非截骨入路内固定术治疗。比较两组的临床效果。结果 研究组的临床病情控制总有效率高于对照组(P<0.05)。两组健侧及患侧复位前、复位后3个月后的Baumann角、提携角比较,差异无统计学意义(P>0.05);研究组的肘关节功能恢复时间短于对照组(P<0.05)。治疗前,两组的肘关节屈伸活动度、旋转活动度、Broberg-Morrey评分比较,差异无统计学意义(P>0.05);治疗12个月后,两组的肘关节屈伸活动度、旋转活动度、Broberg-Morrey评分均优于治疗前,且研究组优于对照组(P<0.05)。结论 与尺骨鹰嘴截骨入路内固定术相比较,内外侧入路非截骨入路内固定术治疗肱骨髁间骨折患者的临床效果更佳,有利于肘关节功能恢复,更值得推荐。

Objective To compare the effects of internal fixation via olecranon osteotomy approach and medial and lateral approach without osteotomy approach in the treatment of humeral intercondylar fractures.Methods A total of 78 patients with humeral intercondylar fractures admitted in our hospital from January 2018 to December 2019 were selected as the research objects and randomly divided into control group and study group,with 39 cases in each group.The control group was treated with internal fixation via olecranon osteotomy approach,and the study group was treated with internal fixation via medial and lateral approach without osteotomy approach.The clinical effects of the two groups were compared.Results The total effective rate of clinical disease control in the study group was higher than that in the control group(P0.05);the recovery time of elbow joint function in the study group was shorter than that in the control group(P0.05);after 12 months of treatment,the flexion-extension range and rotation range of elbow joint and Broberg-Morrey score of the two groups were better than those before treatment,and those of the study group were better than the control group(P<0.05).Conclusion Compared with internal fixation via olecranon osteotomy approach,internal fixation via medial and lateral approach without osteotomy approach in the treatment of humeral intercondylar fractures has a better clinical effect,which is conducive to the recovery of elbow joint function and is more worthy of recommendation.
