浮针疗法对乳腺癌保乳术区硬化影响的临床回顾性研究

Clinical Retrospective Study on the Effect of Fu’s Acupuncture Therapy on the Sclerotic Area of Breast after Breast-conserving Surgery in Breast Cancer Patients

目的观察乳腺癌保乳术区硬化在浮针疗法实施前后程度的变化,探索浮针疗法对该保乳并发症的临床疗效及安全性。方法回顾性分析在2018年6月1日至2019年6月30日用浮针疗法治疗乳腺癌保乳术区硬化患者共52例,采用自身前后比较方法分析评价其疗效。每位患者间隔2~3天进行浮针疗法1次,3次为一个疗程。在第一次治疗前、第一次治疗后,第三次治疗后以及随访(1个疗程结束后2个月),分别观察其乳房硬化程度变化,并进行记录、分级,同时根据硬化分级的变化程度对疗效进行显效、有效与无效评判。运用配对符号秩和检验进行分析,同时分析不良反应记录。结果浮针疗法治疗一个疗程后,显效21例(40.4%),有效30例(57.7%),无效1例(1.9%)。总有效率98.1%。治疗前后分级比较,差异有统计学意义(P<0.05)。除3例治疗后针口局部出现轻微皮下瘀斑外,未出现其他不良反应。结论本研究提示浮针疗法可缓解乳腺癌保乳术区硬化,并具有较好的持续性、稳定性和安全性。

Objective To observe the changes of the sclerosis degree in the breast-conserving operative area of breast cancer patients before and after treatment with floating acupuncture therapy,and to investigate the clinical efficacy and safety of floating acupuncture therapy for breast-conserving surgery complications.Methods From June 1,2018 to June 30,2019,a total of 52 patients with sclerosis in the breast-conserving operative area of breast cancer treated with floating acupuncture therapy were retrospectively analyzed with the self-comparison method for evaluating the effectiveness.The patients received floating acupuncture therapy once every two or three days,three times as a course,and all of the patients completed one course of treatment.The changes in the degree of breast sclerosis were observed,recorded and graded before the first treatment,after the first treatment,after the third treatment and at the follow-up(two months after the end of the one course).And the effectiveness of treatment was judged as significantly effective,effective and ineffective according to the changing degree of sclerosis.Paired signed rank sum test was used for analysis,adverse events were then recorded.Results After a course of treatment,21(40.4%)case were significantly effective,30(57.7%)cases were effective and one(1.9%)case was ineffective.The total efficiency was 98.1%.The pre-and post-treatment of grading comparison showed that the difference was statistically significant(P<0.05).There were no other adverse reactions except three cases with slight subcutaneous ecchymosis at the needling mouth after treatment.Conclusion It is indicated that floating acupuncture therapy can alleviate the sclerosis of the breast-conserving operative area of breast cancer patients,and the therapy has good sustainability,stability and safety.
