多发性骨髓瘤腹泻的认识及中医治疗

Acquaintance of the Refractory Diarrhea Induced by Multiple Myeloma and Its Treatment with Traditional Chinese Medicine

多发性骨髓瘤是临床常见的血液系统恶性肿瘤,目前西医治疗方案以化疗、靶向治疗及干细胞移植为主,而这些治疗均会导致腹泻严重不良事件,给患者造成极大的痛苦或导致治疗中断,影响预后;而部分患者因自身骨髓瘤的髓外侵袭及淀粉样病变,也会导致长期腹泻的发生,给诊断及治疗造成一定困难。本文通过对多发性骨髓瘤的腹泻类型、相关病因病机及典型病例的分析认识,提出此病的治疗思路需根植于温补肾阳,并附医案以供临床参考。

Multiple myeloma is a common malignant tumor of the blood system in clinical practice.Till now,multiple myeloma is primarily treated by chemotherapy,targeted therapy and stem cell transplantation.Unfortunately,the above therapies have a tendency to lead to severe adverse events of diarrhea,which will cause great pain of the patients or lead to the interruption of the treatment,and affects the prognosis.Some patients will also experience diarrhea due to extramedullary invasion and amyloidosis of the myeloma,which will also cause certain difficulties in diagnosis and treatment.Based on the analysis of diarrhea types,related etiology,pathogenesis and typical cases of multiple myeloma,this paper proposes that the treatment for multiple myeloma should be rooted in warming and nourishing kidney yang.
