影响2型糖尿病患者内脏脂肪蓄积的相关因素

Related factors affecting visceral adipose accumulation in type 2 diabetes mellitus patients

目的探讨影响2型糖尿病(T2DM)患者内脏脂肪蓄积的因素,为预防2型糖尿病患者发生内脏脂肪蓄积提供理论依据。方法选取2019年09月~2020年10月在西南医科大学附属医院内分泌科住院的2型糖尿病患者。根据纳入和排除标准,筛选出符合条件的2型糖尿病患者530例。根据内脏脂肪检测结果,将纳入者分为内脏脂肪正常组及内脏脂肪蓄积组。采用Logistic回归分析影响2型糖尿病患者发生内脏脂肪蓄积的影响因素。结果两组患者临床资料比较结果显示性别、年龄、收缩压、舒张压、BMI、皮下脂肪面积、FPG、INS、FCP、2hCP、ALT、AST、GGT、BUN、Cre、UA、TG、HDL-C、尿微量白蛋白/尿肌酐、HbA1C、HOMA-IR差异有统计学意义(P<0.05)。Logistic回归分析结果显示性别、年龄、BMI、皮下脂肪、BUN、TG、尿微量白蛋白/尿肌酐与2型糖尿病患者内脏脂肪蓄积显著相关(P<0.05)。结论性别、年龄、BMI、皮下脂肪、BUN、TG、尿微量白蛋白/尿肌酐与2型糖尿病患者内脏脂肪蓄积有显著相关性。

Objective To investigate the factors affecting visceral adipose accumulation in type 2 diabetes mellitus and provide theoretical basis for preventing visceral adipose accumulation in type 2 diabetes mellitus.Methods Subjects were selected from patients with type 2 diabetes who were admitted to the endocrinology department of the affiliated hospital of southwest medical university from September 2019 to October 2020.According to the inclusion and exclusion criteria,530 eligible patients with type 2 diabetes were screened.According to the results of visceral adipose test,the subjects were divided into normal visceral adipose group and visceral adipose accumulation group.Logistic regression was used to analyze the influencing factors of visceral adipose accumulation in type 2 diabetes mellitus patients.Results The comparison of clinical data between the two groups showed statistically significant differences in gender,age,systolic blood pressure,diastolic blood pressure,BMI,subcutaneous adipose area,fasting plasma glucose(FPG),insulin(INS),fasting c-peptide,2 h c-peptide,alanine transferase(ALT),aspartate aminotransferase(AST),glutamine transpeptidase(GGT),blood urea nitrogen(BUN),creatinine(Cre),uric acid(UA),triacylglycerol(TG),high density lipoprotein cholesterol(HDL-C),uric microalbumin/urinary creatinine,HbA1 C,and HOMA-IR(P<0.05).Logistic regression analysis showed that gender,age,BMI,subcutaneous adipose,BUN,TG,uric microalbumin/uric creatinine were significantly correlated with visceral adipose accumulation in type 2 diabetes mellitus patients,and the difference was statistically significant(P<0.05).Conclusion Gender,age,BMI,subcutaneous adipose,BUN,TG,urinary microalbumin/urinary creatinine were significantly correlated with visceral adipose accumulation in type 2 diabetes mellitus patients.
