MR与多层螺旋CT对诊断颈动脉狭窄和粥样硬化斑块的临床应用价值分析

Analysis of Clinical Application Value of MR and Multi-slice Spiral CT in the Diagnosis of Carotid Artery Stenosis and Atherosclerotic Plaque

目的:分析颈动脉狭窄及粥样硬化斑块应用磁共振(MR)与多层螺旋电子计算机断层扫描(CT)诊断的临床价值。方法:回顾性分析2020年11月-2021年11月徐州医科大学附属医院收治的80例脑血管缺血性疾病患者的资料,所有患者均进行了MR及多层螺旋CT检查,并以DSA诊断结果作为判定标准,比较MR与多层螺旋CT诊断颈动脉狭窄和粥样硬化斑块的临床价值。结果:在颈动脉狭窄诊断中,MR检查诊断准确度为80.00%(64/80),多层螺旋CT检查诊断准确度为76.25%(61/80),两种检查方式诊断结果准确度比较,差异无统计学意义(χ^(2)=0.114,P=0.735);粥样硬化斑块诊断中,MR诊断准确度为82.50%(66/80),多层螺旋CT诊断准确度为57.50%(46/80),MR诊断结果准确度高于多层螺旋CT,差异有统计学意义(χ^(2)=8.333,P=0.004)。结论:MR与多层螺旋CT在颈动脉狭窄诊断中无明显差异,均具有良好的诊断价值,但在颈动脉粥样硬化斑块诊断中,MR诊断价值优于多层螺旋CT,临床应用价值更高。

Objective:To analyze the clinical value of magnetic resonance (MR) and multi-slice computed tomography (CT) in the diagnosis of carotid artery stenosis and atherosclerotic plaques.Method:A retrospective analysis was made of 80 patients with cerebrovascular ischemic disease admitted to the Affiliated Hospital of Xuzhou Medical University from November 2020 to November 2021.All patients underwent MR and multi-slice spiral CT examinations,and the diagnosis results of DSA were used as gold standard,the clinical value of MR and multi-slice spiral CT in the diagnosis of carotid artery stenosis and atherosclerotic plaque were analyzed and compared.Result:In the diagnosis of carotid artery stenosis,the diagnostic accuracy of MR examination was 80.00% (64/80),and the diagnostic accuracy of multi-slice spiral CT examination was 76.25% (61/80),there was no statistical difference between the diagnostic methods (χ^(2)=0.114,P=0.735);in the diagnosis of carotid atherosclerotic plaque,the diagnostic accuracy of MR was 82.50% (66/80),and the diagnostic accuracy of multi-slice spiral CT was 57.50% (46/80),the accuracy of MR diagnosis was higher than that of multi-slice spiral CT,the difference was statistically significant (χ^(2)=8.333,P=0.004).Conclusion:There is no significant difference between MR and multi-slice spiral CT in the diagnosis of carotid artery stenosis,and both have good diagnostic value,but in the diagnosis of carotid atherosclerotic plaque,the diagnostic value of MR is better than that of multi-slice spiral CT,and the clinical application value is higher.
