MRI增强扫描在高级别脑胶质瘤中的应用价值 被引量:1

摘要 目的:研究MRI增强扫描在高级别脑胶质瘤中的应用价值。方法:选取2018年3月-2021年3月福建中医药大学附属第二人民医院收治的88例疑似高级别脑胶质瘤患者作为研究对象。所有患者均给予MRI增强扫描及CT扫描。以术后病理学检测结果为金标准,分析MRI增强扫描与CT扫描的诊断结果。分析MRI增强扫描与CT扫描高级别脑胶质瘤的影像学特征。结果:MRI增强扫描诊断符合率为88.64%,敏感度为90.00%,特异度为83.33%;CT扫描诊断符合率为70.45%,敏感度为72.86%,特异度为61.11%。MRI增强扫描诊断符合率和敏感度均高于CT扫描(P<0.05)。在MRI增强扫描中,高级别脑胶质瘤存在信号不一致性,一般以T_(2)WI高信号、T_(1)WI低信号最为常见,T_(1)WI也存在高信号的表现;肿瘤与周围脑组织界限不清晰,特别对于高级别脑胶质瘤而言,周围水肿十分明显,信号常常伴有不均匀表现,同时合并重度水肿、坏死,占位征象十分明显,并且患者肿瘤周围存在出血征象,可见含铁血黄素的沉积。在CT扫描中,平扫时,高级别脑胶质瘤多数患者伴有低密度混杂表现,伴有不规则和环状等密度区域、不均匀强化,患者伴有瘤内囊变和轻度脑水肿,增强扫描后多数患者伴有不同形态强化情况,以环状强化较为常见,存在外壁光滑、环壁厚薄不匀、内壁不规则情况。结论:MRI增强扫描在高级别脑胶质瘤中具有显著的诊断价值,能显示肿瘤的形态,诊断符合率高,为后期治疗提供依据。 Objective:To study the application value of MRI enhancement scan in high grade brain glioma.Method:A total of 88 patients with suspected high grade brain glioma admitted to the Second People’s Hospital Affiliated to Fujian University of Traditional Chinese Medicine from March 2018 to March 2021 were selected as the study subjects.All patients were given MRI enhancement scan and CT scan.The results of post-operative pathological examination were taken as the gold standard,and the diagnostic results of MRI enhancement scan and CT scan were analyzed.The imaging features of high grade brain gliomas by MRI enhancement scan and CT scan were analyzed.Result:The diagnostic coincidence rate of MRI enhancement scan was 88.64%,the sensitivity was 90.00%,and the specificity was 83.33%;the diagnostic coincidence rate of CT scan was 70.45%,the sensitivity was 72.86%,and the specificity was 61.11%.The diagnostic coincidence rate,sensitivity of MRI enhancement scan were higher than those of CT scan(P<0.05).In MRI enhancement scan,there was signal inconsistency in high grade brain glioma,generally T_(2)WI high signal and T_(1)WI low signal were the most common,and T_(1)WI also had the manifestation of high signal;the boundary between the tumor and the surrounding brain tissue was not clear,especially for high grade brain glioma,the surrounding edema was very obvious,and the signal was often accompanied by uneven performance,at the same time,it was complicated with severe edema and necrosis,and the space occupying sign was very obvious.There were bleeding signs around the tumor of the patient,and the deposition of hemosiderin could be seen.In CT scan,at plain scanning,most patients with high grade brain gliomas were accompanied by low-density mixed performance,irregular and annular etc density areas and uneven enhancement,and patients were accompanied by intratumoral cystic change and mild brain edema.After enhanced scanning,most patients were accompanied by different forms of enhancement condition.Annular enhancement was more common,with smooth outer wall,uneven thickness of annular wall and irregular condition of inner wall.Conclusion:MRI enhancement scan has significant diagnostic value in high grade brain glioma,which can show the shape of the tumor,with high diagnostic coincidence rate.It can provide basis for late treatment.
