不同固定方法对股骨粗隆间骨折患者治疗的效果观察

Effect of Different Fixation Methods on Patients with Femoral Intertrochanteric Fracture

目的:探讨不同固定方法对股骨粗隆间骨折患者治疗的效果。方法:选取2020年6月-2021年6月罗定市人民医院收治的股骨粗隆间骨折患者80例,根据入院顺序分别纳入观察组与对照组,每组40例。观察组接受股骨近端防旋髓内钉(PFNA)方法固定,对照组接受股骨近端锁定钢板(PFLP)固定。记录两组的术中情况、术后情况及髋关节Harris功能评分。结果:观察组手术时间短于对照组,术中出血量少于对照组,差异有统计学意义(P<0.05)。观察组住院时间和骨折愈合时间短于对照组,住院费用低于对照组,术后引流量少于对照组,差异均有统计学意义(P<0.05)。观察组并发症发生率为5.00%,低于对照组的22.50%,差异有统计学意义(P<0.05)。观察组髋关节Harris功能评分总分为(87.95±6.95)分,高于对照组的(80.71±5.21)分(P<0.05)。结论:两种固定方式均能够较好地完成股骨粗隆间骨折固定。但与PFLP固定方式相比,PFNA更具优势,具有手术时间短,创伤小,术后恢复快,骨折愈合早的优点,且并发症更少,更利于患者髋关节功能恢复,在没有禁忌证的情况下可以优先考虑该固定方式。

Objective:To explore the effect of different fixation methods on patients with femoral intertrochanteric fracture.Method:A total of 80 patients with femoral intertrochanteric fracture treated in Luoding Municipal People’s Hospital from June 2020 to June 2021 were selected and included in the observation group and the control group according to the admission order,with 40 cases in each group.The observation group was fixed with proximal femoral nail anti-rotation (PFNA),and the control group was fixed with proximal femoral locking plate (PFLP).The intraoperative condition,postoperative condition and Harris function score of hip joint were recorded.Result:The operation time in the observation group was shorter than that in the control group,and the amount of intraoperative bleeding was less than that in the control group,and the differences were statistically significant (P<0.05).The hospitalization time and fracture healing time in the observation group were shorter than those in the control group,and the hospitalization cost was lower than that in the control group,the postoperative drainage volume was less than that in the control group,and the differences were statistically significant (P<0.05).The incidence of complications in the observation group was 5.00%,lower than 22.50% in the control group,the difference was statistically significant (P<0.05).The total score of Harris function score of hip joint in the observation group was (87.95±6.95) points,which was higher than (80.71±5.21) points in the control group (P<0.05).Conclusion:Both fixation methods can better complete the fixation of femoral intertrochanteric fracture.However,compared with PFLP fixation,PFNA has more advantages.It has the advantages of short operation time,less trauma,rapid postoperative recovery and early fracture healing,and fewer complications.It is more conducive to the recovery of hip function.In the absence of contraindications,PFNA fixation can be given priority.
