摘要
目的探讨MRI检查对骨肉瘤的诊断效能。方法选取93例骨肉瘤患者,均接受CT和MRI检查。以病理学检查为金标准,评估CT、MRI检查对骨肉瘤的检出情况,并比较CT与MRI对骨肉瘤征象及骨肉瘤浸润范围的检出情况。结果病理检查结果显示,93例骨肉瘤患者中成骨型骨肉瘤43例,溶骨型骨肉瘤36例,混合型8例,皮质旁骨肉瘤6例。CT检查可见骨肉瘤软组织高密度影,多数股骨皮质见密集筛洞,髓腔内肿瘤邻近皮质中断处与肿块间相连接。MRI检查骨肉瘤为T1加权成像(T1WI)低信号、T2加权成像(T2WI)信号不均匀或高信号。CT检查与MRI检查对骨肉瘤不同病理类型的检出率比较,差异均无统计学意义(P﹥0.05)。MRI检查对骨肉瘤征象中肿瘤骨的检出率低于CT检查,差异有统计学意义(P﹤0.05)。MRI检查对骨骼肌浸润的检出率明显高于CT检查,其显示的肿瘤纵向、横向范围均明显大于CT检查,差异均有统计学意义(P﹤0.01)。结论MRI检查可精准评估骨肉瘤的浸润程度,其显示的肿瘤纵向、横向范围均明显大于CT检查,可为患者手术方案制订提供参考信息。
Objective To explore the diagnostic efficacy of MRI examination for osteosarcoma.Method A total of93 patients with osteosarcoma were selected and underwent CT and MRI examination.Using pathological examination as the gold standard,to evaluate the detection of osteosarcoma by CT and MRI examination,and compare CT and MRI in the detection of osteosarcoma signs and the extent of osteosarcoma infiltration.Result Pathological examination results showed that among the 93 patients with osteosarcoma,43 were osteogenic osteosarcoma,36 were osteolytic osteosarcoma,8 were mixed,and 6 were paracortical osteosarcoma.CT examination showed high density shadows in soft tissue of osteosarcoma,and dense sieve holes were seen in most femoral cortex.MRI examination of osteosarcoma was T1 weighted imaging(T1WI)low signal,T2 weighted imaging(T2WI)signal inhomogeneous or high signal.There was no significant difference in the detection rates of different pathological types of osteosarcoma between CT and MRI(P>0.05).The detection rate of tumor bone in the signs of osteosarcoma in MRI examination was lower than that in CT examination,and the difference was statistically significant(P<0.05).The detection rate of skeletal muscle infiltration by MRI was significantly higher than that of CT,and the range of longitudinal and transverse tumor were significantly larger than those of CT,and the differences were statistically significant(P<0.01).Conclusion MRI examination can accurately assess the degree of infiltration of osteosarcoma,and the range of longitudinal and transverse tumor are significantly larger than CT examination,which can provide reference information for the formulation of surgical plans for patients.
