延长放射治疗线在乳腺癌简易乳腺托架固定放射治疗中的应用

Application of Extended Radiotherapy Line in Simple Breast Bracket Fixation Radiotherapy for Breast Cancer

目的探讨延长放射治疗线在乳腺癌简易乳腺托架固定放射治疗中的应用价值。方法回顾性分析2019年11月至2021年12月在医院行手术治疗的92例乳腺癌患者的临床资料,将患者随机分为对照组和试验组,每组46例。两组术后均实施放射治疗,治疗前,对照组不延长放射治疗线行锥形束CT(CBCT)固定扫描,试验组则延长放射治疗线后行CBCT固定扫描,比较两组CBCT摆位误差以及CBCT图像发生身体S形弯的患者比例。结果试验组左右、腹背方向的CBCT摆位误差分别为(2.04±1.63)、(1.89±1.62)mm,均小于对照组的(2.98±1.97)、(2.61±1.55)mm,差异有统计学意义(P<0.05);两组头脚方向的CBCT摆位误差比较,差异无统计学意义(P>0.05)。试验组CBCT图像发生身体S形弯曲患者比例(6.52%)低于对照组(26.09%),差异有统计学意义(P<0.05)。结论延长放射治疗线后,乳腺癌患者左右、腹背方向的CBCT摆位误差均减小,CBCT图像发生身体S形弯曲患者比例降低,且患者左右方向的CBCT摆位误差减小得更明显。

Objective The application value of extended radiotherapy line in simple breast bracket fixed radiotherapy for breast cancer was discussed.Methods With the retrospective analysis of the clinical data of 92 breast cancer patients who underwent surgical treatment in the hospital from November 2019 to December 2021,the patients were randomly divided into control group and experimental group,with 46 cases in each group.Both groups were treated with radiotherapy after operation.Before treatment,the control group received cone-beam CT(CBCT)fixed scan without extending the radiotherapy line,while the experimental group received CBCT fixed scan after extending the radiotherapy line.CBCT setup errors and the proportion of patients with body S-bend on CBCT images were compared between the two groups.Results The positional errors of CBCT left-right and ventral-dorsal directions in the experimental group were(2.04±1.63)mm and(1.89±1.62)mm,respectively,which were lower than those in the control group(2.98±1.97)mm and(2.61±1.55)mm,and the differences were statistically significant(P<0.05).There was no significant difference in CBCT positional error to head and foot direction between the two groups(P>0.05).Compared with the control group(26.09%),the proportion of patients with S-shaped bending in CBCT images in the experimental group(6.52%)was lower,and the difference was statistically significant(P<0.05).Conclusion After the extension of radiotherapy line,with the decrease of the CBCT positional errors of breast cancer patients in the left-right and ventral-dorsal directions,and the proportion of patients,the CBCT positional errors of patients in the left-right direction were reduced more significantly.
