运用“不温则痛”理论辨治痛证探析

Analysis on differentiation and treatment of post-amputation neuralgia based on the theory of "being not warm in qi and blood bringing about pains"

痛证是临床常见多发病,病因多样,病位不同,表现各异。阳气是生命存在的基础,主持着生命活动的全过程。痛证的发生取决于阳气的变化。不温是痛证发病的始动因素,阳气不足使然,日久或易感外邪,或变生内邪,缠绵难愈,治疗不可一以蔽之,当析证审机,把握整体,灵活机法,缓急有度、标本兼顾,以求阴阳调平。孙西庆教授结合时代特点提出的“不温则痛”理论,为临床痛证的诊疗提供了思路,临床可资借鉴。

Pain syndrome is a common and frequently-occurring disease in clinical practice,with various etiologies,different disease locations and different manifestations.Yang qi is the foundation of life,governing the whole process of life activities.In the same way,the occurrence of pain syndrome depends on the change of yang qi.Being not warm in qi and blood is the initiating factor of pain syndrome,which is caused in turn by insufficient yang qi.Over time,one with deficient yang qi may be susceptible to exogenous pathogens,or may breed internal pathogens,which will eventually lead to lingering pains and are difficult to be wiped out all at a time.Accordingly,pains should be treated by differentiating syndromes and recognizing etiologies,adopting a holistic and flexible method,dealing with both chronic and acute symptoms reasonably,treating both manifestations and the root cause,in order to seek for a balance between yin and yang.The theory of"being not warm in qi and blood bringing about pains",put forward by taking into account the characteristics of the times,is expected to provide significant ideas and references for the diagnosis and treatment of clinical pain syndrome.
